Job summary

Beaumond House Hospice Care delivers personalised, high-quality end of life care across hospice, home, and community settings. Patients and families remain at the centre of everything we do. All staff are expected to uphold dignity, safety, confidentiality, and the highest standards of physical, psychological, and spiritual care.

This role sits within the End-of-Life Care Together Alliance and operates across organisational boundaries to deliver safe, coordinated, and person-centred care for people in North Nottinghamshire.

This role is based in Beaumond House Hospice, Newark & community settings across North Nottinghamshire.

Main duties of the job

The post holder works collaboratively with the Beaumond House team and the End-of-Life Care Together Integrated Hub to deliver a consistently high standard of hospice care in line with the organisations philosophy and values.

The role focuses on planning, coordinating, delivering, and evaluating person-centred, evidence-based palliative care for patients, families, and carers.

The post holder supports the effective delivery of the Beaumond House Hospice at Home service with in an allocated case load, working closely with the multidisciplinary team and partner agencies. The role contributes to staff and service development, supports admission avoidance, and supports timely discharge to enable people to remain at home in accordance with their preferences and care plans.

Beaumond House Hospice Care is a registered charity. We provide four In-patient beds, Day Therapy, and Hospice at Home Care. Hospice at Home supports people with palliative and end of life care needs, often in the last weeks and months of life. Our nurses and health care assistants provide assessment, personal care, emotional support, advice, and advance care planning in the persons preferred place of care.

We are contactable 24 hours a day, 7 days per week, to provide a friendly ear, general information or to signpost patients, relatives and carers to appropriate resources and services.

Main Duties

Clinical Care

  • Holistic assessments are undertaken in patients homes, with delivery of individualised nursing care in line with assessed needs and agreed care plans.

  • Care plans and clinical documentation are reviewed, updated, and maintained in accordance with organisational requirements.

  • Safe, effective symptom management and end of life care are provided, including recognition and escalation of deterioration and symptom change.

Integrated Working and Care Coordination

  • Partnership working is maintained with the Integrated Hub, Hospice at Home Lead, Care Navigators, and wider system partners to support coordinated care.
  • Autonomous practice is exercised within scope of professional competence, with timely escalation to the Hospice at Home Lead, on call clinician, or other appropriate professionals when required.

  • Advice, information, and education are provided to patients, families, and carers to support understanding, decision making, and safe care delivery.

  • Understanding (and maintaining) the importance of dignity, privacy, confidentiality, safeguarding, and professional boundaries in care delivery.

    Leadership, Supervision and Workforce

    • Health Care Assistants are supported in delivery of care, including supervision, guidance, and coordination of delegated tasks.

    • Delegation is undertaken appropriately in line with NMC guidance, local policy, and competency frameworks, with oversight and accountability maintained.

    • Role modelling of professional practice is demonstrated, including mentoring and support for development of colleagues.

    • Contribution is made to recruitment, induction, and staff development activity as required.

    • Staffing pressures, capacity risks, and safety concerns are identified and escalated promptly in line with local processes.

    • Support is provided to students and learners in practice in line with placement requirements and competence.

    Safeguarding, Risk and Governance

    • Practice is delivered in accordance with safeguarding adults policies and procedures, with appropriate escalation and referral where required.

    • Risk assessments are undertaken and reviewed, including lone working and environmental risks in the home setting.

    • Accurate documentation is maintained in line with NMC standards, organisational policy, and CQC expectations.

    • Medicines management policies are followed at all times, including safe handling, administration, documentation, and escalation of medicines concerns.

    • Participation is required in audit, incident reporting, learning reviews, and quality improvement activity in line with governance frameworks.

    Bereavement Support

    • Bereavement support is provided to carers and families in line with hospice guidance.

    • Those requiring specialist bereavement support are identified and referred through appropriate pathways.

    Professional Development

    • NMC registration is maintained and practice complies with the NMC Code.

    • Engagement is expected in reflective practice, clinical supervision, and appraisal activity as applicable to bank arrangements.

    • Mandatory and developmental training is completed and maintained, including training relevant to lone working, safeguarding, medicines, and end of life care.

    • Resilience is promoted and emotional wellbeing is supported through appropriate use of supervision and support routes.

    General Duties
