The Hospice at Home Service is an integral part of our Care Services. We provide holistic range of care services for patients in their own home, flexibly to suit their individual needs.

The service includes helping patients with personal care, psychological and spiritual support, assessment of needs and help with advanced care planning and end of life care, whilst providing essential support for carers and loved ones.

Role Purpose

Main Duties

  • Responsible for the management and coordination of the Hospice at Home Service, ensuring seamless day to day delivery of high-quality support and care.
  • Provide line management to the team including HCAs and Registered Nurses. This will include performance management.
  • Continue to develop and innovate the Hospice at Home Service Work in close partnership withall the hospice services and allied professionals in the local community, working collaboratively to achieve outstanding care and outcomes for our patients and carers.
  • Service development- contribute to and motivate others to achieve outstanding hospice provision and work collaboratively with stakeholders and allied professionals.
  • Be an integral part of staff training and development and provide guidance and supervision to all members of the team.

Act as an ambassador, actively seeking opportunities to outwardly promote the service.

General Duties

Skills, Experience and Attributes Required

  • A qualification as a Registered Nurse and experience working at a senior level
  • Qualifications in palliative care or leadership are desirable, but not essential
  • Experience working in palliative care or a passion to work in palliative care
  • Excellent people and communication skills
  • Ability to make appropriate judgements related to symptom management of patients that may involve complex situations, analysing what is needed and developing a care plan
  • Ability to work collaboratively with health, social care and voluntary sector colleagues across primary and secondary care to enhance care and support for patients and their families
  • Ability to develop clinical practice changes arising from updated knowledge, audits and inspections.
  • Evidence of providing leadership and successful team building
  • Effective organisation skills and proven people management experience
  • Demonstrate knowledge of clinical governance and risk management
  • IT skills for use of electronic clinical records and to work in a paper light organisation
  • The ability to work as part of a team and a respect for all other individuals
  • Able to work under pressure and respond to the needs of the service
  • Flexibility, excellent listening skills and an ability to build a rapport with others and a friendly and sensitive approach
