Job summary

The salaried GP will be an integral part of the core multidisciplinary primary care team within the Hammersmith & Fulham Health Centres that is split across two sites at Charing Cross Hospital and Hammersmith Hospital.

The post holder will provide the full range of General Medical Services in collaboration with other healthcare professionals within the Centres. There is an expectation to deliver on national quality markers such as the GMS Quality and Outcomes Framework (QOF), National Service Frameworks targets, and other national and local targets within our GMS contract, through robust quality assurance systems, appropriate use of skill-mix, use of information technology, and by education and development of our staff.

We are looking to recruit a motivated and experienced GP with a range of Clinical experience and skills, for a minimum of 4 sessions per week to ensure we continue to deliver high quality care.This role requires the post holder to be a trained Mental Health First Aid champion or a Freedom to Speak Up Champion.

Main duties of the job

  • Provide a full range of medical services as defined in the GMS contract, additional and enhanced services where appropriate.
  • Make professional, autonomous decisions in relation to presenting problems whether self-referred or referred from other healthcare professionals within the organisation.
  • Assess the healthcare needs of patients with undifferentiated and undiagnosed problems.
  • Screen patients for disease risk factors and early signs of illness.
  • Develop care plans for health in consultation with patients and in line with current practice; disease management protocols, provide counselling and health education.

  • Maximise the effective use of current clinical computer systems. Record clear and contemporaneous consultation notes to an agreed standard.
  • Compile and issue computer generated acute and repeat prescriptions (avoiding handwritten prescriptions whenever possible).
  • To take part in areas including clinical auditing and data collection.
  • Contribute to QOF and Northwest London Standard requirements and recording & code data appropriately
  • Attend team meetings and other clinical meetings as necessary.

  • Work as part of the multi-disciplinary clinical team to support management and target delivery in patient with long term conditions.
  • work flexibly between sites, to provide the best continuity of care for patients.

  • Be responsible for maintaining professional development. Attendance at clinical meetings, staff meetings and multidisciplinary meetings contribute towards CPD. Protected CPD time will be agreed upon appointment.

  • To take part and contribute to clinical auditing and data collection
  • Attend and present at monthly LCW Quality Assurance Group

Stakeholder Engagement & Service Development

  • Key member in the PCN network, supporting the delivery of co-ordinated, high quality care across all network practices
  • Respond to stakeholder enquiries and ensure satisfactory resolution.
  • Support service improvement initiatives and change management projects.
  • Assist in investigating complaints, feedback, and patient safety incidents (PSIRF), ensuring timely action and shared learning.
  • Support LCW in its work to understand and continually improve patient experience through gaining feedback and taking actions to address.

Risk & Resilience

  • Identify and mitigate risks, ensuring they are logged in LCWs quality management system.
