Job summary

The North Hammersmith & Fulham Primary Care Network (PCN) is seeking a highly motivated and experienced Non-Clinical Director to provide strategic, operational, and programme leadership across its nine GP practices serving a population of 58,000 in and around the W12 area of London.

This senior leadership role will be central to supporting the PCNs continued growth and maturity, ensuring delivery of integrated, population-focused, and digitally-enabled primary care services in line with the recent NHS 10 Year Health Plan, North West London ICB priorities, and the transformation to neighbourhood-based integrated teams.

Main duties of the job

The post holder will play a key role in improving health outcomes and reducing inequalities for a diverse and deprived local population, coordinating service transformation, workforce development, stakeholder engagement, and delivery assurance.

Working closely with the PCN Clinical Director, GP partners, practice teams, NHS and community partners, and LCW UCC (the employing organisation), this is an exceptional opportunity to shape the future of primary care in North Hammersmith & Fulham and establish a strong foundation for sustainable, high-quality services.

London Central West Unscheduled Care Collaborative (LCW UCC) is the employing organisation for this role within North Hammersmith & Fulham PCN. LCW also runs the Partnership for Health GP Practice in collaboration with Imperial NHS Trust.

Established in 1994, LCW UCC is a not-for-profit social enterprise providing health and social care for the benefit of the community.   • Providing strategic and operational leadership, aligning PCN priorities with the NHS 10 Year Health Plan, NWL ICB objectives, and local population health needs.

  • Championing population health management and place-based working at PCN/neighbourhood level, focusing on prevention, early intervention, and integrated care.

  • Driving PCN development and transformation, embedding new care models including enhanced access hubs, same-day triage, and wraparound services.

  • Supporting the operationalisation of Integrated Neighbourhood Teams (INTs).

  • Overseeing delivery of PCN specifications and ARRS workforce schemes.

  • Monitoring and reporting on PCN performance, using data and analytics to improve access, manage demand, and achieve better patient outcomes.

  • Building strong partnerships with practices, community providers, local authorities, and patients, ensuring meaningful engagement and representation at borough and system forums.

  • Driving digital innovation across the PCN, supporting digital-first approaches, online triage, the NHS App, shared care records, and secure interoperable systems.

  • Overseeing workforce planning and development, supporting recruitment, training, and delivery of ARRS roles, while promoting an inclusive culture.

  • Maintaining professional standards through appraisal, mandatory training, and continuous personal development.

  • 37.5 hours per week
