Job summary
Our Safe
Harbour service seeks to reach out to deprived, marginalised and minority communities
who have not traditionally accessed hospice care.
We are looking for a self-motivated individual with good people skills to
join our team to engage with underserved communities and patients, with a
specific focus on areas of socio-economic deprivation.
Main duties of the job
The successful candidate will provide a visible presence in these
communities through attending groups and events and running evidence based
short term projects. They will engage
with a wide variety of local stakeholders and develop a knowledge of local
services to secure the best outcomes for patients. They will work as part of a
multi-disciplinary team to support patients to access services to ensure they receive equitable care.
To support delivery of the Safe Harbour objective to improve access to hospice services for underserved groups, with a specific focus on socio-economically deprived communities.
To raise awareness of hospice services through engaging with local groups, members of the public, and service providers.
To identify and work with assets in the local community and develop a database of local services.
To support Safe Harbour patients and families to access services and receive equitable care as required.
To co-produce events, activities and projects with Safe Harbour communities

Job responsibilities
To plan and generate work with underserved communities and write up results.
To be a visible presence in deprived communities. This may require working from a range of places including cafes, shared working spaces, community centres and libraries.
Spend time with local groups, organisations and people in deprived communities to learn about the communitys needs and raise awareness of Hospice services.
To contribute to developing an evidence base that demonstrates the requirements for services that our underserved populations require as part of the Safe Harbour team.
To support with the management of Safe Harbour volunteers.
To work as part of the Hospice and Hospice in the Home Multi-disciplinary teams.
To visit patients and families from underserved groups in their home, or mutually agreed location.
To keep accurate patient records.
Travel independently, in own car to patient homes and various community locations.
To develop a network of contacts and database of services to support Safe Harbour patients.
Representation at internal and external meetings and networking events.
Contributing to St Helenas EDI group and internal awareness raising initiatives.
Setting up and running evidence-based community groups and activities.
Assisting with funding applications.
