Job summary
We have an opportunity for a full-time Care Co-ordinator to join our Fair Havens Community Team, located at our Fair Havens Hospice, adjacent to beautiful Priory Park in Southend on Sea.
As this role supports a 7-day service, there is a requirement that candidates be available to work on a rostered basis, across the 7 days, including weekends and bank holidays.

Main duties of the job
You will work as part of the wider team of coordinators within our Fair Havens Community Team, to ensure that our processes and HRAS referrals are streamlined with robust internal pathways, as well as undertaking administrative duties as required.

Job responsibilities
- To be responsible for data collection for all aspects of placements utilising SystmOne records and computerised databases
- To work closely with clinical staff within the Core teams; to deliver the above service and to escalate clinical concerns and sourcing difficulties to the Rapid Access, and to the Head of Adult Services/Community Lead.
- To support the Rapid Assessment and Discharge Team with administrative support as required
- Through telephone, virtual and face-to-face discussions, provide information to patients with palliative and end of life care needs, supporting their families and carers, liaising with clinical professionals and referring on as necessary to appropriate health and social care partners
- To support the Hospice in building strong relationships with external partners and stakeholders and promote the good reputation of the Hospice e.g. local trusts, ICB, health and social care partners, businesses and voluntary organisations

- To collaborate with Fair Havens Community Lead Nurse in streamlining processes and procedures within the team using quality improvement methodology and meaningful clinical outcome measures
- To ensure contracts are prepared and signed for following standard operating procedures
- To negotiate best value for money regarding placements
- To escalate to clinical teams or your line manager any clinical or operational issues in a timely fashion
- To input the required administrative information
- To ensure personal care data entry and collection and support presentation of such data to meet quality requirements
- To train other relevant staff e.g. Navigators and Administrators, in Nursing Home procurement and data entry requirements
- To work with the Service Lead to establish, implement and participate in systems and processes that support the service
