Job summary

An opportunity has arisen for an experienced senior nurse to join our Children and Young Person Community Team.

You will be managing a caseload of patients requiring long-term support, with complex needs, travelling throughout Essex to their homes, or working within the hospice setting.

Main duties of the job

  • Assess plan, implement, and evaluate care in conjunction with the young adult, family, and carers.
  • Provide holistic nursing care for children, young adults and their families within the community or hospice setting.
  • Carry out risk assessments of the family home and working environment.
  • Be involved in the referral process by attending the multidisciplinary team (MDT) meeting and completing a holistic assessment with the patient, family, and carer. Presenting the outcome to the MDT.
  • Prevent hospital admission through early detection of symptoms by providing symptom management advice in conjunction with the appropriate healthcare professional.
  • Support the CYP, their family and carers with end-of-life care planning to include preferred priorities of care.
  • To participate in young people's activity groups and other groups/ activities as required
  • Implement strategies to provide timely, appropriate, and equitable services to young adults and their families.
