Farleigh Hospice is a high performing organisation providing specialist palliative and end of life care for people with life limiting illness in the Mid Essex region. Our team at Farleigh has grown and evolved to meet the changings needs of the community we serve, and we are looking for highly motivated Community Team Manager to join our senior clinical team.
Main duties of the job
You will be a compassionate leader with excellent communication skills. You will be a clinician registered with a relevant professional body e.g.NMC, HCPC, GMC with ideally experience in palliative care and managing multidisciplinary teams. You will have the ability to problem solve, remain calm under pressure, and manage competing workloads. Farleigh Hospice is as an integral part of the emerging Integrated Care System.
You will report to the Head of Community Services and have the opportunity to develop your leadership skills and act at a strategic level. You will continue to practice clinically in your designated field.

To support and lead multidisciplinary teams to deliver an efficient holistic palliative care service, that can respond flexibly to patient and carer needs and is focused on achieving agreed outcomes for the patient
To manage and inspire a multidisciplinary workforce from novice to expertto work as one team
To actand practiceas a senior clinician within the community teams and, as required, the wider organisation
To innovate and streamline community processes and services by embedding quality improvement methodologyincluding clear outcome measures to produce efficient and highly performing community services
It is anticipated thatthe role willincludeclinical work in your set discipline (20%), management and leadership(80%) including supporting/deputising for the Head of Community Services when required

- Extensive post qualification experience, with 3 years at a senior clinical level

- A track record of delivering excellent services that meet diverse user needs
- Experience of service development, flexible approaches and introducing innovative ideas to achieve outcomes for patients
- Experience of collaborative working, internally and externally to achieve results
- Experience of working withend of life orlong-term conditions
