Job summary

You will undertake comprehensive health assessments both face to face and over the telephone, including adults and paediatrics; practicing autonomously whilst demonstrating a person-centred approach to care delivery. Within your professional boundaries you will assess, diagnose, plan, implement and evaluate treatment/interventions. Provide care for patients presenting with undifferentiated and undiagnosed conditions and complex needs. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly. You will practice autonomously and prioritise, organise and manage own workload in a manner that maintains and promotes quality. Take responsibility for own learning and performance, acting as a positive role model.

Advanced Clinical Practitioner

Are you an Advanced Clinical Practitioner? Primary Care 24 (PC24) are excited to share that we are looking for an ACP to work within our GP Practices in Primary Care. We have set shifts available over our two clusters around Merseyside.

Salary: £25072.80 - £25748.21 per annum

Location:

Cluster 1 15 Sefton rd.,Litherland, Seaforth, Netherton health centre.

Cluster 2 Maghull, Hightown, Thornton Health centre, Crosby/Crossways, North Park.

Shifts:

Monday 10.30-6.30pm & Friday 8am -4pm (Cluster 1)

Wednesday 8am - 4pm (Crosby/Crossways)

What will you be doing

  • You will undertake comprehensive health assessments both face to face and over the telephone, including adults and paediatrics; practicing autonomously whilst demonstrating a person-centred approach to care delivery.

What do I need

  • Full NMC Registration with no restrictions
  • MSc in Advanced Clinical Practice is desirable; candidates working towards this qualification or who have relevant experience are encouraged to apply.
  • Non-Medical Prescriber V300
  • Clinical Examination and Diagnostics Level 7
  • 2+ years of experience working within a GP Practice.
