Job summary

Advanced Clinical Pharmacist - Bank Are you an Advanced Clinical Pharmacist? We are looking for an Advanced Clinical Pharmacist who will join us on a bank basis. You will focus on developing and delivering the medicines optimisation and safety agenda alongside a team of clinical pharmacists. Key responsibilities include repeat prescribing, managing long-term conditions, conducting clinical medication reviews, and clinical audit and safety work. The role requires expertise in medication management and patient care. Salary: £30.00 per hour Location: Remote - Liverpool What will you be doing Processing repeat and non-repeat medication requests Clinical medication reviews with patients to optimise their treatment. Support the development and implementation of prescribing policies. Reconcile medicines after hospital discharge and ensure patients receive required treatments. Monitor and review prescribing patterns to ensure compliance with the Pan Mersey Formulary. Answer medicine-related queries from healthcare staff and patients. Contribute to audits and quality improvement initiatives. Provide clinical advice. Support public health campaigns, including immunisations and flu vaccination initiatives. Ensure compliance with CQC standards and health, safety, and infection control policies. Work collaboratively with the wider healthcare team, including GPs, nurses, and other professionals, to deliver exceptional patient care. Advocate for patients, ensuring clear communication about their treatment options. Contribute to fostering a supportive and efficient team environment.
