This is an exciting time for PC24 as we are transforming and implementing new models of care across our practices. We are seeking to appoint Bank Nurse Practitioners to cover our 12 practices and our new purpose-built primary care hub offering a family-friendly service to our patients across Liverpool and Sefton.

Main duties of the job

As a PC24 Nurse Practitioner you will have:

  • Full NMC Registration with no restrictions
  • Non-Medical Prescriber V300
  • Clinical Examination Level 7
  • Diagnostics Level 7
  • Experience of working in primary care

Job responsibilities

Key responsibilities

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undifferentiated and undiagnosed conditions and complex needs.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
  • Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national/local protocols and within scope of practice.
  • Work with patients to support compliance with and adherence to prescribed treatments.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.

Primary Care Shifts are available between the hours of 08:00 and 18:30 across all 12 of our locations in Sefton and Liverpool. We are a patient centred organisation and therefore require flexibility to cross cover across cluster sites as required:
