This is an exciting time for PC24 as we are transforming and implementing new models of care across our practices. We are seeking to appoint 3 Lead ACPs (8B) Advanced Clinical Practitioners to cover our 12 practices and our new purpose-built primary care hub offering a family-friendly service to our patients across Liverpool and Sefton.

Main duties of the job

The successful post holder will be to primarily deliver senior clinical decision-making level of care to patients within the primary care. In addition, they will lead on the evolving programme of advanced practice from training through to senior clinical level of practice in primary care services.

Job description

Job responsibilities

  • The Strategic Development and Leadership of ACP's in primary care, junior clinical staff, junior nursing staff.
  • Delivery of an approved advanced practice curriculum (national or local in absence) for trainee practitioners.
  • Contribute to PC24 strategies surrounding performance and improvement.
  • Contribute to work force planning and considerations around organisational systems affecting resilience and retention.
  • To provide a governance structure to the training and development of advanced practice across primary care
  • Oversight of budgets for advanced practice within primary care
  • Develop robust clinical streams, alongside colleagues, aimed at safe patient care, experience and efficiency.

Advanced Clinical Practice is an evolving area within acute medicine and across the integrated and urgent care specialties. Drawn generally, but not exclusively, from a background of nurses, paramedics and physiotherapists.

The post holder with have a demonstrable clinical portfolio that evidences clinical skills equivalent to that of a senior clinical member of staff. Their clinical role will involve the independent and autonomous practice of history taking, clinical examination, clinical investigations and interpretation and subsequently the diagnosis and management planning of the differentiated and diagnosed patient as well as the undifferentiated and undiagnosed primary care patients.

The successful applicant with be an established leader able to demonstrate success in service design and delivery relevant to primary care. They will already be at senior practitioner level in an established service.

The successful applicant must have completed an MSc in Advanced Clinical Practice and have consolidated fully academically and clinically.

The successful applicant will be responsible for the leadership and strategic development, across primary care , of all advanced (ACP) practice staff, both trainees and qualified, as well as supporting educational supervisors and engaging directly with Higher Education Institutes (HEIs) for the delivery of academic modules.

The successful applicant must have an in-depth understanding of the advanced practice apprenticeship. They must also engage fully with Health Education England and support any future relevant curriculums and credentials that are released.

The successful applicant must be able to demonstrate the understanding of the relationships between the ACP and their consultant bodies and they must be engaged fully in this key relationship. They must also be heavily involved in service development of all areas within the specialty concentrating around patient safety, experience, efficiency and effectiveness aspects involving care.
