Job summary
We have an exciting opportunity to join Primary
Care 24 (PC24) as a Lead General Practice Nurse. This role has been developed
to work as part of the senior nursing team to support the development and
expansion of our primary care division within PC24. We are looking for a senior
leader with experience in Primary Care who can support and develop our teams
and new models of care.
Main duties of the job
Clinically,
the focus of the role is the delivery of evidence-based practice for patients
with long-term conditions and management and preventative nursing interventions
to all patients. As an autonomous practitioner they will be responsible for the
care delivered, demonstrating critical thinking and skills in clinical
decision-making. They will work collaboratively with the whole general practice
team to meet the needs of patients, supporting the delivery of policy and
procedures, and providing leadership and direction for the nursing team. The post holder will deliver a 70% direct clinical
care component to their role and 30% related to leadership, clinical
innovation, service development, CPD, education, audit with a requirement to
maintain clinical leadership throughout. This may change depending on the
requirements of the role and service needs and determined in
conjunction with the senior Nursing and management teams.

The post holder is responsible for ensuring the delivery of safe and
effective nursing care to the whole practice population. As the team leader for
the nursing team, the post hold holder is accountable for the nursing service
delivery. They will lead and manage the proactive and planned sector of the
PC24 primary care model with an emphasis of quality outcomes framework (QOF)
requirements. They will work in
conjunction with the Deputy Director of Nursing (DDON) Heads of Service (HOS), Practice
Managers, Clinical Leads and GPs to deliver the practice priorities delivering quality
patient care. Working
with the senior nursing and quality team the post holder will lead and monitor
improvements in the quality of care through clinical audit, evidence-based
practice, clinical supervision, teaching, supporting professional colleagues
and the provision of skilled professional leadership. The post holder will lead
the implementation of relevant clinical guidelines into practice and ensure
that there is standardisation of practice across all PC24 practices.
