Job summary

TABA (Tyldesley, Ashton, Boothstown, Astley) Primary Care Network are seeking to recruit a Band 6 equivalent nurse to work across its member practices. This is an exciting opportunity to work in an energetic and innovative Primary Care setting. Although practice nurse experience is desirable it is not essential if you have experience of managing long term conditions e.g. Diabetes, Respiratory etc. This may be an ideal opportunity for an experienced nurse wishing to start a career in Primary Care. Training will be provided.

Main duties of the job

Trainee Enhanced practice nurses have the following key responsibilities in relation to delivering health services -

Works as part of an MDT within the PCN; consults with patients, their family, and the multi-professional team to undertake assessments of patient need and devise and evaluate complex care plans; teaches and advises patients and their families on how to manage their condition or support the multi-disciplinary team to do so.

Provides an excellent Primary Care nursing service to the population served by TABA PCN practices. This includes -

Long term condition reviews (e.g. Diabetes, Asthma, CHD, COPD)

Perform Health checks

Assist in and performing routine tasks related to patient care as directed by senior clinicians

Cytology

New Patient Medicals

ECGs

Asthma/Peak Flow Readings/Spirometry

Venepuncture

Hypertension Management

Routine immunisations/Childhood immunisations

Chaperoning and assisting patients where appropriate who are being examined by another clinician

Assisting GPs with minor surgery and coil fittings

Requesting pathology tests, for example urine culture, swabs

Coronary Heart Disease and Contraception.

Follow agreed clinical protocols with referral to senior nurses or GPs as appropriate.

Mentoring and developing the junior nursing team.

Job responsibilities

Screen patients for disease risk factors and early signs of illness

Be able to assess patients both remotely and in person

Assess and manage people with long-term conditions

Develop with the patient an on-going care plan for health

Discuss patient-related concerns with the relevant staff, and where required, you will be supported to follow appropriate safeguarding procedures

Issue and administer medication and vaccinations within the relevant Patient Group Directions, or, if qualified and appropriate, via Independent Prescribing

Work as part of a diverse team, collaborating with colleagues across the Network to provide the best care to our patients

Ensure safe handover of care within and outside the GP Practices as appropriate.

Contribute to the supervision, training, development and coaching of other staff through peer-support, ensuring a safe and high-quality service is delivered, promoting agreed medical and emergency activities and development programmes.

Provide practice nurse activities in a primary care setting to support the GPs, holding clinics and visiting patients within their own homes.

Actively pursue policies that will promote healthy lifestyles for patients: promote health education and participate in relevant sessions within the practice.

Ensure the clinical computer system is kept up to date with accurate details recorded and amended.

Maintain and clean equipment

Maintain Nurses rooms, stocking, and rotating items as required

Chaperoning and assisting patients who are being examined by another clinician.

Attend and participate in any Practice/PCN meetings when required.

Have knowledge of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines

Correctly and safely manage specimens including collection, labelling, handling, use of correct and clean containers, storage, and transport arrangements
