Job summary

To support our Primary Care Network (PCN) Multi-Disciplinary Team in the delivery of nursing services TABA PCN has an exciting opportunity to recruit an NMC Registered Nurse Associate who is enthusiastic, forward thinking, committed and motivated with excellent communication skills to join our PCN and multiple practice nursing teams. This is a great opportunity for Nurse Associates wishing to apply their skills in a primary care setting.

This role will assist clinical staff across the PCN in the provision of treatment, preventative care and health promotion. You will work independently as a member of the Nursing Teams, under the leadership of a Registered General Nurse and must work at all times within your own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC).

You will arrange clinical investigations, including X-rays, pathology tests and others as indicated to ensure accurate diagnosis and safe and effective condition management.

Main duties of the job

The Nurse Associate will be an integral part of our PCN Team and practice clinical teams to complement the role of the GP and other Health Care Professionals providing safe, high quality care to our patient population.

Key Clinical Responsibilities:

Provide continuing direct and indirect care to patients across the practice population.

Provide clinical observations. phlebotomy, flu vaccinations, ECGs, BPs and venepuncture, and other designated clinical tasks as required.

Assisting in wound care management following patient care plans

Monitoring and support of the management of patients with long-term conditions / chronic diseases, using a holistic patient centred approach.

Support patients to improve and maintain their mental and physical health and wellbeing

Assist with the co-ordination of Infection Control policy and audit.

Chaperone patients when required.

Support registered nurses to enable them to be able to focus on the more complex clinical care

Contribute to the delivery of integrated care.

Promote health and well-being to all patients, for example undertaking the NHS health checks, Hypertension reviews, Prediabetes checks.

Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services.

Undertake mandatory training as requested by the PCN

Undertake training for specific duties as required by the PCN

About us

TABA PCN is a Primary Care Network working across 11 practices in the Tyldesley, Atherton, Boothstown and Astley areas covering a population of 59,000 patients. We are a dynamic, forward looking PCN with a growing team of Additional Roles staff such as pharmacists, paramedics, physios, community link workers, care co-ordinators and mental health workers working across the PCN. You would be part of this expanding, innovative team to improve outcomes for the people we serve through sustainable and high-quality general practice provided collaboratively with member practices.

TABA PCN works in collaboration with Health First ALW in the delivery of these services. Health First is a GP federation, they are a not for profit social enterprise, supporting and developing PCNs and GP practices.
