Are you ready to take the next step in your nursing career and play a key role in delivering high-quality care, then we would love to hear from you!Willen Hospice is seeking a Clinical Nurse Specialist (CNS) to join our specialist multi-professional team, supporting patients with complex specialist palliative care needs on a 12 month fixed term/secondment basis.

In this autonomous role, you will assess, plan, deliver, evaluate, and manage care for patients within their own homes, coordinating and prioritising appropriate admissions into our In-Patient Unit (IPU) when necessary. You will demonstrate best practice, provide expert education and support across health and social care settings, and champion high-quality, holistic palliative care.

Main duties of the job

  • Manage and prioritise a specialist caseload for Willen at Home, working closely with the wider Hospice multidisciplinary team.
  • Provide a responsive service working with local Primary Care Teams and Community Nursing Services, crossing organisational boundaries as needed.
  • Deliver specialist advice on symptom control, psychological support, and holistic care planning for patients and families.
  • Liaise effectively with community, acute, and tertiary care teams to ensure continuity and collaboration in patient care.
  • Educate and support hospice staff, community nurses, and healthcare professionals through formal and informal teaching programmes.
  • Lead and contribute to service development, audits, and research projects, enhancing the Willen at Home service in Milton Keynes.

What Youll Need:

  • Have a minimum of 5 years post-registration experience, with at least 2 years in cancer and/or palliative care.
  • You will be able to demonstrates a passion for service and professional development and delivering high-quality care.
  • You will Ideally be a qualified Non-Medical Prescriber (NMP) or willing to work towards this.
  • You must be confident working independently in the community, holding a valid driving licence and access to a vehicle.
  • Have flexibility to work across day and weekend shifts and take part in an out of hours on call rota.

1. Function as part of the Willen at Home service and as part of the wider Hospice

Multidisciplinary team. Managing and prioritising a case load to best meet the specialist needs of all patients and families accessing the service.

2. Provide a service through Willen Hospice working with the local Primary Care Team and Community Nursing service; respond to the needs of the patient, working across organisational boundaries as required.

3. Assess the wider needs of the patient and their family providing specialist advice and information on symptom control and psychological support issues, referring to other specialists and support networks as required. If you are a qualified Non-medical prescriber, prescribe in accordance with NMC guidelines and local guidelines including Willen hospice NMP policy.

4. Maintain good liaison with community and acute/tertiary hospital staff maintaining communication with key professionals involved in the patients care across the health and social care settings. Promote a collaborative and co-operative working relationship with peers and multi-professionals contacted in the course of work.

5. Act as an educator in relation to specialist palliative care patients, hospice nursing staff and the community nursing team. Participate in the development and delivery of formal and informal teaching programmes for health care professionals and allied professionals across the health care community.

6. Be proactive in the development of the Willen at Home service in the Milton Keynes area working with the Manager, community palliative care team and other providers.

7. Work with the Willen at Home team in the implementation of audit/research projects and disseminate findings through the Audit and Quality Committee. Proactively participate in research projects as an individual and within the team. Be actively involved in clinical audit within the clinical and service provision agenda, providing direction in service development.

8. Participate in the On-Call Rota, providing out of hours support to the In-Patient Unit Teams.

9. Support the work of the Hospice through an annual involvement in at least one extra-curricular activity such as a Fundraising or Marketing event; for example, a visit to a local school, or an event where the service is promoted or income generated.
