Job summary

Paramedic practitioners work independently within their scope of practice in the community, using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems to assess, and manage patients presenting with acute presentations which include minor illness, or injury, abdominal pains, chest pains and headaches. They are health professionals who practice at an advanced level having the capability to make sound judgements in the absence of full information and to manage varying degrees of risk when there are complex, competing or ambiguous information or uncertainty.

Main duties of the job

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment [including prescribing medications following policy, patient group directives, NICE [national] and local guidelines and local care pathways] or make necessary referrals to other members of the primary care team.

Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCNs social prescribing service, and where appropriate, other community or voluntary services.

Perform specialist health checks and reviews within their scope of practice and in line with local and national guidance

Perform and interpret ECGs

CORE DUTIES AND RESPONSIBILITIES

This list of duties and responsibilities, which follows, represents a broad range of responsibilities, which may be required to be undertaken either routinely or periodically, with consultation and agreement by the post holder. The role may require the ability to flex delivery to cover 8am -8pm over a seven-day week to meet Network need.

Responsibilities underpinning the role

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment [including prescribing medications following policy, patient group directives, NICE [national] and local guidelines and local care pathways] or make necessary referrals to other members of the primary care team.

Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCNs social prescribing service, and where appropriate, other community or voluntary services.

Perform specialist health checks and reviews within their scope of practice and in line with local and national guidance

Perform and interpret ECGs

Perform investigatory procedures as required, and undertake the collection of pathological specimens including intravenous blood samples, swabs, and other samples within their scope of practice, and within line of local and national guidance.

Support the delivery of anticipatory care plans and lead certain services (e.g. monitoring blood pressure and diabetes risk of elderly patients).

Provide an alternative model to urgent and same day GP home visit for the network.

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, and patients with complex needs

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Diagnose and manage both acute and chronic conditions, integrating both drug- and nondrug-based treatment methods into a management plan

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

Develop and facilitate a good working relationship with the GP Practices within the Primary Care Network.

Work closely with Rochdale Health Alliances Senior Management Team

Communicate at all levels across organisations ensuring that an effective, person-centred service is delivered.

Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.

Communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings, and Job treatment choices

Plan and organise own workload, including audit and project work

To record personally generated information and maintain a database of information relating to the work done in the practices. Personally generated information includes information and records relating to audit and clinical work undertaken by the post holder, reference notes relating to clinical/technical information, etc. The database includes maintaining up-to-date, detailed records of all work done in the practices for which the post holder is accountable (done by the post holder or others)

To maintain registration as a Paramedic and comply with appropriate professional codes

As appropriate to the post, to maintain and develop professional competence and expertise, keep up to date with medical/therapeutic evidence and opinion, and local and national service, legislation and policy developments, agree objectives and a personal development plan and participate in the appraisal process

To attend local, regional and national meetings of relevance by agreement

To undertake any other duties commensurate with the post holders grade as agreed with the post holders line manager

  • Have completed a 2 years Consolidation of Learning period as a newly qualified paramedic
  • Have completed a further 3 years experience as a Band 6 [or equivalent] paramedic
  • Evidence of enhanced clinical assessment
  • An appreciation of the nature of GPs and general practices
  • Excellent interpersonal, influencing and negotiating skills
  • Excellent written and verbal communication skills
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients)
  • Good IT skills
  • Able to obtain and analyse complex technical information
  • Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or
  • GPs when appropriate
  • Able to work under pressure and to meet deadlines
