Job summary

To be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients at the Practice.

Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population. The post holder will have the opportunity to shape and develop within their position while contributing to the growth of the practice.

The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health promotion, well women and well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

Main duties of the job

To provide a clinically effective practice nursing service to address the health needs of the practice population.

To assess, plan and provide healthcare and nursing services to the practice patients ensuring they are effective, relevant and of a high standard in providing optimum levels of care. Services provided to include:

o Management of patients with chronic diseases

o Provision of contraceptive advice and assist GPs with contraceptive services.

o Taking blood and urine samples, other specimens, and swabs

o Carrying out Spirometry/Reversibility / ECG testing when required

o Offering specialist information and advice in areas such as blood pressure, weight control and smoking cessation advice.

o Suture removal / redressing following minor surgery performed in the practice.

o Administering travel immunisations and offering travel healthcare advice

o Providing advice to patients in respect of their continuing medical and nursing needs

o Administering of gonadorelin analogue treatment, including provision of Zoladex and Prostap

o Provision of ring pessary service

Job description

Job responsibilities

To work towards achieving the targets laid down in the Quality and Outcomes Framework / Core Plus / IIF

To be involved in the development of care guidelines / protocols in collaboration with the GPs for use in practice

Referring to other members of the practice team as necessary

To work in close collaboration with other primary care nurses, allied professionals, GPs, and practice staff to ensure continuity of patient care.

Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule.

Act as a chaperone when required.

Select, carry out and interpret results of specific pathology and other tests.

Carrying out seasonal clinics e.g., influenza

To assist other clinicians in the event of any emergency or resuscitation incidents.

To take the lead on Infection Prevention and Control carrying out 6 monthly audits using national guidance.

Any other duties as deemed appropriate by the Practice Manager.

Administration Duties

To maintain accurate patient records and enter onto the computer using agreed templates and snomed codes.

Ensure accurate completion of all necessary documentation associated with patient health care.

To collect data and participate in regular practice audit and research projects.

To maintain own annual registration with the NMC and adhere to Code of Professional Conduct

To ensure own revalidation is completed on time to continue smooth working practice.

To check all Emergency Drugs Boxes on a weekly basis and ensure all items are replaced when expired / used.

To check emergency resus equipment on a weekly basis.

To carry out Infection Prevention and Control inspections monthly and producing an IPC Audit on an annual basis and reporting on any infection control issues highlighted throughout the year.

To develop and review audit protocols and process for the practice, effectively utilising the audit cycle.

To develop, review and implement nursing protocols in conjunction with the Directors

Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA)

Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation.

Any other duties as deemed appropriate by the Practice Manager or Nursing GP Lead.
