We are excited to be able to have an experienced ACP join our group of GP practices to support patients within our care homes, delivering high quality planned and proactive care to avoid unplanned hospital admissions, as well as working within our GP practices alongside primary care staff and with our home visiting team.

This role will allow the successful candidate to spend the quality time needed with patients to ensure a personalised care approach with shared decision making when discussing and treating their healthcare needs. The role will support our GP practices with the ongoing management of care home residents by reviewing and amending residents' RESPECT paperwork and Personalised Care and Support Plans, as well as undertaking care home rounds and supporting with any multi-disciplinary team meetings.

Main duties of the job

Main Duties:

  • Working collaboratively with the GPs and care homes to meet the patient needs whilst demonstrating critical thinking in the clinical decision-making process. The role allows spending quality time with the patients to offer the care and attention they need.
  • Carry out clinical nursing practice at an advanced level using expert clinical skills, critical judgement & knowledge to deliver appropriate and safe care.

  • Independently prescribe and review appropriate medications, with support from the Clinical Pharmacy Team, to support medication adherence.
  • Co-ordinate the planning and delivery of care, ensuring involvement from the patients and carers.
  • Working in a multi-disciplinary team, delivering safe and high-quality care.
  • Delivery of care in line with NSF, NICE guidelines and evidence-based care
  • Triage and treat patients, performing specialist assessment of their needs and making necessary referrals to other members of the primary and secondary health care teams as appropriate.
  • Request diagnostic investigations and interpret within agreed protocols, eg x-ray, blood tests, scans, etc.
  • Assess, diagnose and treat patients in a community setting
  • Refer patients to other members of the multi-disciplinary practice teams, secondary care and other organisations as necessary.
  • Communicate confidential and sensitive information to patients, relatives and carers in relation to their condition.
  • Contribute to local and national practice targets, eg QOF and prescribing incentive scheme.

Wakefield North Primary Care Network has six GP practices based in Castleford, Featherstone and Normanton with a supportive team of primary care staff and additional roles to work with our patients, which include GPs, nurses and a pharmacy team to name a few.

JOB SUMMARY:

This is a new role within our PCN for an experienced ACP to support patients within our care homes, delivering high quality planned and proactive care to avoid unplanned hospital admissions, as well as working within our GP practices alongside primary care staff and with our home visiting team. This role will allow the successful candidate to spend the quality time needed with patients to ensure a personalised care approach is taken when discussing and treating their healthcare needs.

To work within their professional boundaries providing first point of contact for patients with undifferentiated and undiagnosed health care needs, including telephone triage and face to face consultations. There may be the need to perform home visits as well along with acting as the link and providing care for patients residing in the PCNs Care Homes.

To work collaboratively with the healthcare team, meeting the needs of the patient whilst demonstrating critical thinking in the clinical decision-making process. This will include history taking, clinical assessment and diagnostic skills in partnership with the practices in developing the role further.

Will have a knowledge of long-term conditions.

Independently prescribe (where applicable to the role) and review appropriate medications in accordance with local policies and evidence based medical practice and work collaboratively with the wider team to support patients and to promote medication adherence.

To order investigations, follow-up results and arrange appropriate management including seeking advice where appropriate.

Providing face-to-face, acute medical treatment for care home residents seeking a care primary care service.

To work with a multidisciplinary team including community and voluntary sector to improve patient care.

Actively taking a personalised care approach and population centred care approach to enable shared decision making with the presenting person.

To support the GP practices with the ongoing management of care home residents by reviewing amending residents RESPECT paperwork and Personalised Care and Support Plans as well as undertaking care home rounds and supporting with Multi-Disciplinary Team meetings.

QUALIFICATIONS AND COMPLIANCE:

The successful candidate will have a Post Graduate Degree Qualification in their field, Clinical Practice or equivalent qualification, with evidence of academic study and practical experience of situations that are typically encountered in a primary care setting.

You will be registered with a professional body. It is a requirement for the candidate to have attained a post graduate qualification in Prescribing or be working towards qualification.

CLINICAL ASSESSMENT:

To provide a first point of contact for patients with undifferentiated and undiagnosed health needs at the most appropriate time and place.

Provides and receives highly complex information; develops communication skills/present complex information.

To provide advanced assessment skills in history taking and clinical examination to formulate a diagnosis and initiate an appropriate plan of care, including referral to other health and social care professionals, as necessary.

Instigate and interpret diagnostic investigations as appropriate.

Provide patient and carer information, health education, and health promotion as appropriate.

Prescribe appropriate medication and preparations within existing legislative framework in relation to evidence-based practice, NICE guidelines and considering the financial implications of cost-effective prescribing (if relevant to your specialism).

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and within scope of practice (if relevant to your specialism).

ACUTE & CHRONIC DISEASE MANAGEMENT:

Work with all GPs and other members of the practice as directed in developing protocols for diagnosing and managing minor illnesses, and for chronic disease management etc.

Work with the nursing, clinical pharmacy, and medical team in providing pro-active management of patients with chronic diseases in accordance with NSF and local requirements.

Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

PROMOTE HEALTH:

Enable individuals, families, groups, and communities to address issues that affect their health and social wellbeing.
