Clinical Pharmacist - Independent Prescriber

Job summary

The role will involve monitoring our patients with Alzheimer's disease or dementia with Lewy bodies who have been commenced on medication, to oversee medication titration and stabilization, and to provide advice regarding the choice of alternative medication where necessary. Based at ReMind UK, this will primarily be via telephone consultations with our varied patient group, who often also have polypharmacy and significant co-morbidity. You will be liaising with patients, their carers and their community health care team. You will be supported by our friendly team of medical and nursing experts.

There may be some flexibility with salary depending on experience.

Main duties of the job

  • Undertake medication reviews for patients who have been commenced on dementia medication and are requesting repeat prescriptions
  • Oversee medication titration and stabilization before handing prescribing over to the primary healthcare team according to ReMind UK protocols
  • Answer patient queries and concerns about their medication and any possible side effects, including changing the prescription where necessary
  • Prescribing in a cost-effective manner
  • Advising the primary healthcare teams on options for deprescribing or substitution recommendations if there are medications affecting their cognition
  • Liaise with the primary healthcare teams regarding patient contacts and any other changes to medication dose
  • Maintaining excellent medical records.

About us

ReMind UK is an independent charity and an internationally renowned centre for research, diagnosis and treatment of neurodegenerative diseases. Our research aims to improve life for people with dementia and their families and carers, and to find drug and non-drug treatments for people with conditions such as Alzheimer's disease and other diseases predominantly affecting older people. Our contracted NHS Memory Clinic service, paid for memory assessments, and allied activities also provide support to patients, families and carers. We are currently in an ambitious phase of development to increase significantly our clinical and academic research programme. We work closely with the Universities of Bath, Bristol and other research institutions, both nationally and internationally, and with the Royal United Hospital.

CLINICAL RESPONSIBILITIES

  • Maintain standards in accordance with the General Pharmaceutical Council
  • Be familiar with all ReMind UK policies and Standard Operating Procedures (SOPs) and comply with these at all times, and assist in ensuring the SOPs are kept up to date
  • Help to ensure that patients and carers are as fully informed as possible about their illnesses and benefits to which they are entitled
  • Willingness to help with and give presentations at support courses
  • Promote the work of ReMind UK to visitors, including other health professionals and members of the public, and represent ReMind UK at external functions
  • Take responsibility for on-going personal development and education
  • Share skills and contribute to training new nursing, medical and other staff
  • Contribute to educational sessions, e.g. Journal Club

OTHER RESPONSIBILITIES
