Job summary

Triage and Deployment NurseAs part of our transformation into the hospice of the future, that puts the patient at the heart of everything we do, a fantastic opportunity has opened. This is an exciting chance to help shape change as we become more sustainable as a charity.

The postholder will work collaboratively within the Care Coordination Centre. The Care Co-ordination Centre is at the heart of Community Specialist Palliative Care services. It combinesRapid Response Team, Clinicians of the Day and the Triage and Deployment Team. Its role is to ensure thatreferrals are handled efficiently and patients are seen by the most appropriate clinician/practitioner for their needs.

Main duties of the job

You will work as a triage deployment nurse supporting the triage of all patients that are referred to the hospice ensuring they are routed to the most appropriate service. You will be supporting the use of our deployment tool working with the rapid response service ensuring that patients are allocated promptly based on need. You will also support the senior deployment nurse in the coordination of patients to be admitted to our inpatient unit.
