North London Hospice, a registered charity, has been the leading provider of end-of-life care in North London since 1984, providing specialist care to patients at home, in the community and on our Inpatient Unit in Finchley. Main duties of the job

The Patient and Family Support Team provide psychosocial care to patients, families, and their carers pre & post bereavement.

The PAFs team act as advocates for people with advanced life limiting illness and their family / carers, aiming to enable individuals to make informed choices to manage their health, wellbeing, and lives independently for as long as possible.

This role will require you to work as an autonomous practitioner within a multi-disciplinary team (MDT) and provide a specialist palliative social work service to patients, their families and carers.

You will work with emotional and relationship complexity, acting as an advisor to other members of the MDT on social care issues.

To be successful in this role, you will have:

  • A degree in Social Work or equivalent qualification
  • Registered with Social Work England
  • Proven experience of assessing/planning person-centre interventions
  • Proven experience of working with adults at risk/ adult safeguarding
  • Experience of working in a multi-disciplinary team
  • Experience of supporting people through issues of loss
  • Knowledge of current legislation and social policy relevant to role
