We're now looking for a Band 7 Occupational Therapist to join us as a key contributor to our discharge and transition planning function. Main duties of the job
What You'll Do
Provide advanced OT input into complex discharge planning
Support the development of structured transitional programmes
Work closely with MDTs and external partners to unblock barriers and improve flow
Contribute to service improvement projects focused on progression and discharge
Supervise and develop the OT workforce in discharge focused practice
Lead on outcome measurement and evidence based pathway development

Who You Are
A confident, skilled OT with strong experience in mental health or secure settings, excellent clinical reasoning, and a passion for improving patient pathways and transitional care.
