We're now looking for a Band 7 Occupational Therapist to join us as a key contributor to our discharge and transition planning function. This is a brilliant opportunity to be part of a service that's evolving fast, where your clinical expertise will help shape smoother, safer and more consistent patient journeys.

Main duties of the job

What You'll Do

Provide advanced OT input into complex discharge planning

Support the development of structured transitional programmes

Work closely with MDTs and external partners to unblock barriers and improve flow

Contribute to service improvement projects focused on progression and discharge

Supervise and develop the OT workforce in discharge focused practice

Lead on outcome measurement and evidence based pathway development
