We are looking for a passionate and skilled Coordinator with experience within health and social care along with knowledge of discharge legislation
Main duties of the job
In this role, you will be an integral part of our positive, committed team across the Birmingham division. You will play a key role in supporting safe, timely and effective patient progression across the Birmingham site. Working closely with the Discharge Liaison Nurse, the postholder will coordinate discharge activity, strengthen patient flow, and contribute to reducing length of stay through proactive planning, communication and partnership working.
You will support multidisciplinary teams MDTs, external agencies and internal services to ensure that every patient has a clear, well managed pathway and that delays are identified, escalated and resolved
As part of our forward thinking service, you will play an active role in liaising with internal and external partners including Reach Out, Home Treatment Teams, CMHTs, EIS, commissioners and local authorities whilst facilitating discharge planning meetings and ensuring actions are completed. You will Support the Birmingham sites improvement journey by contributing to projects focused on flow, pathway redesign and reducing length of stay, whilst identifying opportunities to streamline processes and enhance patient experience
