Job summary

We have an exciting opportunityto join ourteam as Head of Integrated Care. The team are committed to delivering the highest possible standards of respite, palliative and end of life care to babies, children and young people. Working at Derian House will involve working with a team of dedicated professionals at all levels and also provides the opportunity to make a real difference to the babies, children and young people we support and their families.

Main duties of the job

To provide visible, credible, compassionate and values based clinical leadership and guidance to support, manage and develop the Hospice clinical team to provide excellent care. To monitor and utilise resources effectively to ensure the needs of babies, children, young people and their families are met by providing excellent quality and safe care. To promote compassionate care across teams, underpinned by robust Governance processes to constantly improve clinical care. To facilitate a culture of openness, transparency and ensure all staff have a voice and contribute to service delivery and development.

Derian House is rated Outstanding by the CQC; Derian House provides symptom management, palliative and end-of-life care, as well as short break respite care to babies, children, young people and their families with life limiting conditions across Lancashire, South Cumbria and Greater Manchester. Supported by our dedicated multi-disciplinary teams we are able to provide a wide range of support and care in a variety of settings, including in-house in our Hospice in Chorley and in the wider community.

Job responsibilities

Main Purpose of Post:

Lead, manage, and support the Hospices services across in-patient services and community to promote and develop a cohesive and strong team.

Support the Director of Clinical Services/Deputy Director of Clinical Services to champion the delivery of excellent care of babies, children, young people and their families providing compassionate leadership, and retaining a focus on engagement and co-production within the services.

Provide effective leadership to the Care team promoting the highest standards of safe, high quality care and an effective learning environment for all staff.

Ensure a robust professional approach to service development and improvement.

Work in collaboration with the Director of Clinical Services/Deputy Director of Clinical Services and colleagues to promote a positive working culture.

Act as a key member of the Leadership & Management Team contributing fully to the development delivery and achievement of all clinical objectives.

Ensure good clinical governance processes are embedded across services and underpin practice.

Deputise for the Deputy Director of Clinical Services as required.

Promote best practice and implement innovations in care within all services.

Ensure high quality, safe patient care is delivered by an appropriately skilled and

educated workforce, through effective leadership by setting standards and performance monitoring of those standards.

Work collaboratively with all stakeholders including Integrated Care Boards, Acute Hospital Trusts, Tertiary Hospitals, GPs, Community Teams etc. to ensure a high standard of quality care delivery including - safety, effectiveness and patient experience, learning from feedback and patient and public involvement strategies.

Qualifications

Essential

  • Current registration in the NMC register in Childrens nursing.
  • First Level Degree education
  • Evidence of continued professional development.
  • Recognised teaching / assessing qualification
  • Leadership & Management qualification or equivalent relevant experience

Desirable

  • Evidence of Masters level learning (or willing to work towards).
  • Enhanced Advanced communication Skills Training.
  • Non-Medical Prescribing qualification
