Job summary
We are looking for someone who can deliver a 6 hour clinic on a Monday or a Wednesday at the Ormskirk PCN Hub at Hants Lane, as part of the Ormskirk Atrial Rapid CVD Prevention Service.
This prevention service is a one stop shop approach to investigate irregular pulse rates and identify Atrial Fibrillation early and diagnose, inform and manage stroke risk in patients providing better quality care.
The Practice Nurse will be responsible for the delivery of practice nursing services delivering care within their scope of practice. You will work as part of the multidisciplinary team GP, Paramedics, Clinical Pharmacists and Care Co-ordinators.
The main focus of the role will be carrying out 45 minute consultations with patients referred into the service including 12 Lead ECGs and Bloods, part of the patient pathway. You may be required to deliver other clinics to support primary care in Ormskirk for example vaccination clinics, well women services and long term condition reviews.
There will be an option to deliver additional hours at part of the West Lancashire Access Services.
Main duties of the job
45 minute Atrial Fibrillation Consultations including:-
Clinical history taking to exclude Symptomatic patients requiring Urgent Ambulatory Referral or AED Referral.
History and symptom identification of arrythmia and to identify potential causes of AF. (e.g. alcohol, Amphetamine or cocaine use, excess stimulant consumption.)
CHA2DS2VASc Score
Cardiac examination [MEWS score, Pulse, BP, JVP, Peripheral Oedema, Auscultation (Heart Murmur), Ausculation (Pulmonary - Heart Failure)12 lead ECG
Blood test.
Send ECG reports via email if Care Co-ordinators are unavailable
Identify, manage and support patients at risk of developing long term conditions, preventing adverse effects on the patients health
Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
Provide vaccination and travel medicine services
Provide chronic disease clinics, delivering patient care as necessary, referring patients into secondary/specialist care as required
Maintain accurate clinical records in conjunction with extant legislation
Deliver well woman/man clinics
Implement vaccine programmes for adults and children
Carry out cytology screening including smears
Recognise, assess and refer patients presenting with mental health needs
Support patients in the use of their prescribed medicines or over the counter medicines (within scope of practice)
Support the clinical team with all safeguarding matters in accordance with local and national policies
About us
Employment will be with Out of Hours West Lancashire CIC Limited (OWLS). We host a number of roles on behalf of the three West Lancashire Primary Care Networks (PCNs) and their member practices. This role will be hosted on behalf of Ormskirk PCN.
OWLS is a small GP owned and led not-for-profit primary care organisation run by GPs and health professionals. As a not for profit organisation all the money we generate through service contracts is reinvested in providing patient care.
We were founded in the 90s by a small group of GPs to provide high quality out of hours services. In 2017, we became the West Lancashire GP Federation. The Federation supports and provides services for West Lancashire GP practices and provides a vehicle to bid for and provide innovative primary care services.
Ormskirk Primary Care Network brings together the four GP practices in Ormskirk to deliver primary care services at scale.
