Clinical Lead for Community Beds

Job summary

We are seeking an experienced GP to take on the role of Clinical Lead for Community Beds. This is a clinical leadership role responsible for the medical model within a community bed-based service delivered in partnership with Leeds City Council and Leeds Community Healthcare Trust across Leeds.

The post-holder will act as the responsible clinician across multiple bed bases (currently four), providing clinical leadership, governance oversight, and direct patient care. The service supports patients who are typically stepped down from acute hospital settings, with some step-up admissions from the community.

The role combines clinical leadership (8 hours per week) with hands-on clinical delivery (minimum 1 day per week), working within a multidisciplinary team including GPs, Advanced Clinical Practitioners, resident doctors, geriatricians, pharmacists, and pharmacy technicians.

Main duties of the job

Clinical Leadership & Service Delivery:

  • Act as the responsible clinician for patients within the community bed bases.
  • Provide clinical leadership for the medical model across all bed bases.
  • Deliver direct patient care within the service (minimum 1 day per week).
  • Support safe, effective admission, management, and discharge of patients.
  • Work collaboratively with MDT colleagues to deliver holistic patient care.
  • Support development and implementation of care pathways, including escalation to geriatricians.
  • Ensure continuity and quality of clinical care across sites.

Clinical Governance, Safety & Quality:

  • Lead on clinical governance for the medical model of the service.
  • Ensure high standards of patient safety, quality, and clinical effectiveness.
  • Oversee clinical audit, quality assurance, and continuous improvement processes.
  • Lead investigations and implementation of learning from incidents and complaints.
  • Ensure compliance with regulatory standards (including CQC) and best practice.

  • Undertake regular ward rounds across community bed bases, providing comprehensive medical reviews of patients.
  • Assess, diagnose, and manage a wide range of acute and chronic conditions, with a particular focus on frailty and complex comorbidity.

Advanced Care Planning & Patient-Centred Care:

Multidisciplinary Team Working:

  • Work closely with nursing, therapy, pharmacy, and medical colleagues to deliver coordinated, holistic care.
  • Actively participate in MDT meetings, case discussions, and care planning processes.
  • Provide clinical advice and support to other members of the team where appropriate.
  • Contribute to a collaborative and supportive team culture.

Medicines Management:

  • Undertake medication reviews in partnership with pharmacists and pharmacy technicians.
  • Optimise prescribing for frail and elderly patients, considering polypharmacy and deprescribing where appropriate.
  • Safely prescribe medications and ensure appropriate monitoring and follow-up.

Discharge Planning & Flow:

  • Work proactively with therapy, nursing and other medical colleagues to minimise length of stay while maintaining safe, high-quality care.
  • Support timely and safe discharge planning from admission through to discharge.
  • Ensure clear, accurate, and timely communication of discharge summaries and plans.
  • Facilitate safe transfer of care back to the patients registered GP and community services.

Partnership & System Working:

  • Work collaboratively with partners including Leeds Community Healthcare NHS Trust and Leeds City Council.
  • Build strong working relationships across organisational boundaries.
  • Contribute to integrated working and service development across the system.

Operational Collaboration:

  • Work closely with operational colleagues to ensure effective service delivery.
  • Support development of pathways, processes, and escalation protocols.
  • Contribute to service performance and improvement initiatives.

Leadership, Workforce & Development:

  • Provide line management to clinical staff within the service.
  • Support recruitment, induction, and ongoing development.
  • Conduct appraisals and manage performance issues where required.
  • Provide mentorship, supervision, and clinical leadership.
  • Actively support training, education, and upskilling of the multidisciplinary team.
  • Foster a positive, collaborative, and high-performing team culture focused on continuous learning.

Support staff within own team with motivation, development and training. Provide support and expertise to own team members.
