Job summary

The NHS Health Check Clinical Development Nurse will provide clinical leadership and oversight for the delivery of the NHS Health Check Programme across Leeds. The role will support the high-quality and consistent delivery of the programme, ensuring that it aligns with national guidance and local population health priorities

Main duties of the job

The post holder will act as a clinical advocate for the NHS Health Check Programme, supporting GP practices to improve the uptake, quality, and outcomes of NHS Health Checks. The role will work in close partnership with the Deputy Service Delivery and Relationship Manager, the Operational Service Delivery & Performance Lead, Public Health, Leeds City Council, and General Practice teams, including administrative staff, Healthcare Assistants (HCAs), Nurses and wider clinical teams.

MAIN DUTIES

Provide clinical leadership and specialist expertise to support the delivery, development, and continuous improvement of the NHS Health Check programme across Leeds.

Promote best practice in cardiovascular disease prevention, early detection, and risk management in line with national guidance, NICE recommendations, and Public Health priorities from Leeds City Council.

Support clinicians and healthcare professionals in delivering high-quality NHS Health Checks through the implementation and ongoing development of the quality assurance framework.

Act as a clinical champion for preventative health, advocating for the value and impact of NHS Health Checks and supporting their integration within wider primary care and Public Health, Leeds City Council initiatives.

Support the development, implementation, and evaluation of NHS Health Check initiatives, pathways, and service improvements to ensure the programme remains responsive to population health needs.

Promote and support quality improvement initiatives aimed at increasing NHS Health Check uptake, improving clinical outcomes, and reducing negative variation through the quality assurance framework.

Review performance and population health data to identify trends, inequalities, and variation in delivery, and work collaboratively with the operational team and delivery partners to implement actions that support service improvement.

Provide clinical insight and practical support to the operational team and general practice teams to help GP practices and Primary Care Networks (PCNs) meet programme Key Performance Indicators.

Work with GP practices and PCNs to review and strengthen Standard Operating Procedures (SOPs) relating to NHS Health Check delivery, supporting the development of consistent and effective processes.

Work creatively with GP practices to identify realistic and practical solutions to barriers affecting NHS Health Check delivery, tailoring support based on individual practice needs, workforce capacity, and local population health data.

Ensure the NHS Health Check programme is delivered in line with national guidance, clinical standards, and local policies.

Support the development, implementation, and review of clinical pathways, protocols, and referral processes associated with NHS Health Checks.

Contribute to internal clinical governance processes, including risk management and quality assurance audits that support and strengthen the organisations governance infrastructure.

Provide clinical advice, guidance, and support to staff delivering NHS Health Checks through Leeds GP Confederations Enhanced Access Service and across wider general practice delivery teams.

Deliver NHS Health Check training and contribute to the development of healthcare professionals working within general practice and wider partner organisations, including third sector organisations, community services, and social prescribing teams.

Promote awareness and understanding of the NHS Health Check programme across primary care and support services, ensuring a consistent level of knowledge and high-quality delivery of the programme.

Support integration between NHS Health Checks and wider prevention programmes such as smoking cessation services, weight management programmes, and the National Diabetes Prevention Programme.

Champion a population health and health inequalities approach by supporting targeted outreach and engagement strategies that improve access to NHS Health Checks for underserved or higher-risk communities.

Engage with a wide range of stakeholders across primary care, Public Health, Leeds City Council, and community organisations, using strong communication and influencing skills to promote the value of NHS Health Checks and inspire partners to maintain prevention as a key priority.
