Job summary

Working within the Service Delivery Team as a Health Coach within the Weight Management Interface Hub, you will support adults and families who are overweight or obese to achieve sustainable lifestyle change through person-centred, weight management programmes. Drawing on your experience of helping individuals with lifestyle modification, you will deliver nutrition, physical activity and behaviour change interventions through group, one-to-one, community and digital formats. You will build strong relationships with diverse communities across Leeds, monitor service user progress using SMART goals, and work collaboratively with partners to reduce health inequalities and improve long-term health outcomes

HOURS OF WORK: Up to 37.5 hours a week

NATURE OF CONTRACT: Fixed term 1 year

ACCOUNTABLE TO: Service Delivery & Relationship Manager

ROLE SUMMARY

Working within the Service Delivery Team as a Health Coach within the Weight Management Interface Hub, you will support adults and families who are overweight or obese to achieve sustainable lifestyle change through person-centred, weight management programmes. Drawing on your experience of helping individuals with lifestyle modification, you will deliver nutrition, physical activity and behaviour change interventions through group, one-to-one, community and digital formats. You will build strong relationships with diverse communities across Leeds, monitor service user progress using SMART goals, and work collaboratively with partners to reduce health inequalities and improve long-term health outcomes

MAIN DUTIES

Working as a Health Coach you will be working with adults and/or families to support, motivate and advise those who are currently overweight or obese.

You will be passionate about working with individuals to begin the life-changing processes necessary for permanent and continued weight loss and weight maintenance.

You will understand challenges and barriers that adults face with relation to their health (both physical and mental) and wellbeing. You will engage with and build a good rapport with adults from varying communities across Leeds.

You will deliver our programmes of weight management as well as supporting the development of new nutrition and exercise programmes that meet the needs of our service users.

The ideal candidate will be passionate about promoting healthy lifestyles and delivering motivational change which is person centered.

You will have a background in health coaching, nutrition, exercise, public health, community health or adult and/or family weight management and be an excellent team player and have a willingness to learn and develop.

Duties and Responsibilities will include;

Service Delivery

Being part of a wider team that will deliver our adult weight management programme including nutrition, behaviour change and exercise delivery to groups and though one-to-ones, in the community and through digital methods such as video and telephone consultations.

Deliver evidence-based education around nutrition, physical activity and behavioural change

Deliver programmes with a person centered approach using skills of positive psychology and motivational interviewing

Monitor service user progress through regular consultation and identifying specific goals for success

Working with individuals and groups using SMART goal setting

Keep detailed consultation notes, recording data accurately, ensuring all forms and other documentation is completed correctly

Be able to organise own workload in order of priority and deadlines, daily administration tasks and other work-related activities.

Undertake and complete all mandatory and additional skills training as required

Service Development

Work with service users and internal/external stakeholders to co-produce meaningful session materials

Contribute to the contract performance by submitting regular case studies to the line manager

Contribute to training and support new staff

Build community links and partnership working to encourage new referrals into the service and maintain close working relationships with other community services.

Relationships

Present service and referral pathway information at stakeholder meetings/community events as directed by the Service Delivery and Relationship Manager

To attend and actively participate in team meetings, such as MDT

To demonstrate personal and professional development in line with ethos of lifelong learning

To maintain statutory and mandatory training in line with the Leeds GP Confederation training matrix
