Job summary
The purpose of the role is to provide high quality specialist palliative nursing care, skills, advice and support to patients, carers and those important to the patient. This role will support the delivery of care across Rennie Grove Peace Hospice Care.
- Salary: £34,685 - £41,798 per annum (pro rata)
- Hours: 30 - 37.5 hours per week

Senior Hospice at Home Nurse/ Paramedic roles available in the following locations:
- Buckinghamshire/Bucks (Chalfont St Giles and South Bucks area)
- North West Hertfordshire/Herts (St Albans and Dacorum area)
- South West Hertfordshire/Herts (Watford and Three Rivers area)
Main duties of the job
You will therefore be responsible to:
- Actively participate in the development of the organisations planned and unplanned community Hospice at Home model of care.
- To work as part of the multidisciplinary team to provide a high standard of individualised patient / family care and support.
- To be responsible for those patients allocated on a daily basis.
- To ensure that excellent standards of support and clinical practice and care are achieved.
- Deliver specialist palliative nursing care to patients to include a multi-disciplinary approach for our higher acuity patients .
- To work with Hospice team and system partners to support delivery of care and enable patients to remain in their preferred place of care
- Improve patient outcomes and experience.
- Act as a Rennie Grove Peace Hospice Care ambassador within the community, actively demonstrating our values and positively promoting the specialist care we provide.
- Work across Rennie Grove Peace Hospice Care geographical area (as required) to support safe and effective service delivery.
