Job summary
To work in children and young peoples homes as part of the hospice at home service. To support the registered nurse in providing and monitoring safe, person-centred care to all babies, children and young people who are receiving care from the hospice at home team, without direct supervision, mainly lone working, with an agreed plan of care in partnership with children, young people and their families and the wider MDT.
Under the direction of the registered nurse, participate in the assessment, planning, implementing and evaluation of nursing care given to babies, children, young people and their families, working in partnership with them.
Actively engage with individuals, their families and/or carers and contribute to risk assessments and care planning, by establishing their needs, wishes, preferences and choices and incorporating these into care planning. Provide a high standard of care to all babies, children and young people.
Main duties of the job
The hospice at home service is working as part of Rainbows hospice for babies, children and young people. Hospice at Home will be supporting end of life and palliative care for babies, children and young people within their own home, allowing them to experience the amazing Rainbows experience on their doorstep, Rainbows wherever you are.
About us
As a hospice at home nursing associate, you will work as part of Rainbows hospice at home team providing clinical care to babies, children and young people with life threatening/life limiting conditions within their own homes, to give parents/carers care and support.
The role includes assessment within your scope of practice, and providing specific, planned day-to-day care to the babies children and young people according to their level of need during Hospice at home sessions.
This will involve duties which cover 7 days per week, with the flexibility to do nights, upon discussion.

- To take personal and professional responsibility for administration, storage and return of all medications in accordance with Rainbows policy.
- To maintain accurate and contemporaneous records according to policy.
- Handle information and data in line with national and local policies and legislation.Communicate sensitive information effectively and improve communication using a range of strategies with regards to person centred care, duty of candour, equality and diversity
- To have a knowledge an understanding of SystmOne.
- To Act independently and in partnership with others to ensure the rights of individuals are not overlooked or compromised.
- To liaise with members of the wider multi-disciplinary health care team and any other agencies as required, to ensure continuity of care for the families.
- To maintain confidentiality at all times in line with the NMC code of professional conduct for nurses, midwives and nursing associates.
- To report to the registered nurse any concerns surrounding the care, condition or behaviour of the baby, child/young person/family.
- To participate in Hospice Open Days and Remembrance Days where required.
- To participate as a team member in the shift rota, working when required.
- To follow the correct procedures for the authorisation and reporting of sickness and absence.
