At St Leonard's Hospice we have a great opportunity for a full time Registered Nurse to join our multi-disciplinary team who provide a high standard of skilled, compassionate nursing care and symptom management to patients within the community.
This is an ideal opportunity for anyone who would like to extend and develop their palliative and end-of-life care skills. You will have the opportunity to work with highly skilled staff across our Hospice@Home, working across a full shift rotation pattern (days, evenings, weekends and bank holidays).

This is a full-time position working 37.5 hours per week.
Main duties of the job
Working in our Hospice@Home team as a Registered Nurse, your main duties will be to:
- work as a member of the Hospice multi-disciplinary team to provide a high standard of skilled, compassionate nursing care and symptom management to patients
- be responsible for the planning, delivering and evaluating the effectiveness of care given to patients
- provide emotional support to patients loved ones
- maintain accurate written records of care, communicating all necessary information to other team members
- create a positive and safe environment for patients
What we need from you:
- previous experience of working as a nurse within a community setting including having excellent nursing skills, record keeping and medicines management
- excellent communication skills and have the ability to be sensitive and empathetic

To
work as a member of the Hospice multi-disciplinary team to provide a high
standard of skilled, compassionate nursing care and symptom management to
patients in the community setting.

provide support to patients, families and carers.
Main
duties and responsibilities
Professional/ Clinical
- Maintain
a multi-disciplinary team approach to the individualised care of the patient
and maintain high standards of nursing care.
- Assess
the needs of the patient and their family taking into account their physical,
psychological, social and spiritual needs.
- Work closely with all members of the
Community Team involved in the patients care and agree our input and level of
involvement.
- Plan, initiate, and constantly evaluate
the effectiveness of care under the direction of the Community Nursing Team and
Hospice@Home Team Leaders.
- Maintain accurate written records of
care, communicating all necessary information to other team members and any
Community Services involved.
- Participate fully in the setting,
implementation and monitoring of formal standards of practice and to be
involved, when required, in clinical governance and audit.
- Comply with the requirements of the
Nursing and Midwifery Council regarding Professional Registration and to
Post-Registration professional education and revalidation.
- Communicate appropriately with all
patients, relatives, and staff, liaise with other departments and agencies, and
attend departmental and interdepartmental meetings as requested.
- Take part in the audit process as
required.
- Participate in full rotation across all shifts.
Administrative/ Managerial
- Maintain
a caring and mutually supportive working environment for all members of staff
and volunteers, being sensitive to the emotional needs of colleagues and giving
appropriate support to them.
- Maintain accurate records regarding the
administration and custody of drugs within the community setting. To comply with all statutory and
organisational policies relating to drug administration and management.
- Maintain high standards of safety within
the home, reporting any hazardous occurrences or equipment.
- Report all accidents, incidents,
complaints, and untoward occurrences, keeping statutory and organisational
records as required.
- Exercise care and economy in the use of
property and equipment.
Education
- Assist
in the teaching of new staff and volunteers so that they are able to function
fully within the team.
- Participate
in the teaching and guidance of visiting professionals eg Student Nurses,
Social Workers and Post Graduate Students.
- Communicate
skills learnt on secondment or study leave to other members of staff.
- Access
and positively engage in role-related e-learning as directed by line managers
and Lead Nurse for Education.
