Do you want to join the St Leonard's Hospice Team increasing the reach of our palliative care services across the local community?

We are recruiting two Wellbeing Coordinators to join the Hospice's Wellbeing & Outreach Team to increase the reach of our palliative care services across all demographics of the local population, helping to ensure that our services meet the needs of the community.

This is a full-time position working 37.5 hours per week between office working hours, Monday to Friday, for a fixed period of 2 years.

Main duties of the job

The main duties of these roles will be to:

  • Create and maintain strong relationships with external organisations, community groups to raise awareness
  • Continue to improve St Leonards understanding of the community we serve and bring back the voice of the community
  • Help break down barriers to accessing palliative care services

Our ideal candidates will:

  • Have experience of providing face to face support to individuals or families in a care or support setting
  • Have excellent interpersonal skills and be able to be sensitive and empathetic
  • Be approachable, honest and trustworthy
  • Be able to deliver multiple priorities simultaneously and to a timeline

As this role will involve joining community groups, it is essential for you to have a driving licence and drive your own car.

To work within the Wellbeing & Outreach team and alongside other internal teams to increase the reach of our palliative care services across all the demographics of the local population, helping ensure the service meets those peoples ongoing needs.

To understand the voice of the community and those with lived experience and bring the learning back to the organisation.

  • Receive highly complex and demanding information and deal with distressing situations in a sympathetic, calm and supportive manner. Escalating issues to the correct services or other health care professionals where appropriate.
  • Support referrals to the Hospices clinical services.
  • Support Wellbeing & Outreach groups such as: Wellbeing Hubs, bereavement groups, disease specific socials and one to one support to service users on a virtual caseload.
  • Be responsible for working alongside and/or supporting volunteers within the team.
  • Manage day to day administrative functions of the service e.g. arranging of meeting/diaries and incoming post. Use a degree of initiative, prioritising workload and reviewing work practices in accordance with service development.
