About us
Hospice at Homes core service is our skilled team of Registered Nurses and Healthcare Assistants who provide exceptional care and support in peoples last year of life, at end of life, also providing additional services such as Lymphoedema, Bereavement and Family Support, Befriending, Occupational Therapy and Complementary Therapy.
We provide individualised care and support to improve quality of life and our end of life care helps facilitate a peaceful and dignified death in the home and support to those who are bereaved.
Our clinical service area covers a mixture of urban and rural communities, a region of approximately 1,500 square miles and a population of 200,000 throughout North and East Cumbria.

Job responsibilities
Responsibility for Patients, Families, and Carers
- Provide direct
nursing care to palliative and end of life patients, including catheter
care, pressure care, simple dressings, and support with daily living.
- Implement
individualised care plans, monitor changes in patient condition, and
escalate concerns to GPs or out of hours services when required.
- Accurately
document all care delivered during and after each visit.
- Offer
emotional, psychological, and practical support to families and carers,
ensuring they feel informed and supported throughout the care process.
- Provide
information on palliative care services, statutory and voluntary support,
and Hospice at Home when requested.
- Instruct
carers on the safe use of equipment such as hoists following professional
assessment, ensuring correct use and reporting any faults appropriately.
Administration and Self Development
- Support
induction of new staff through shadowing opportunities.
- Maintain
compliance with all mandatory training requirements.
- Plan and
organise home visits effectively.
- Participate in
supportive conversations throughout the year.
- Attend monthly
team meetings and complete annual appraisals.
