are looking to grow our Lymphoedema Team with a caring and dynamic individual
who enjoys working in the community.
Would
you like to provide care for patients with Lymphoedema and chronic oedema as
one of the only providers in the area you would be welcomed as part of this
integral team.

settings; assessing complex needs and providing high level care and support to
those receiving palliative care or those who present with chronic conditions?

Main duties of the job
- To maintain expert levels of clinical competence in regard to lymphoedema management
- To ensure a high standard of clinical care based on available evidence, national guidelines and professional standards, supporting more junior staff to do likewise.
- To recognise the need for medical intervention and refer accordingly
- To facilitate referral to other members of the health and social care team as appropriate
- To be responsible for a caseload of patients, including those with a complex presentation, undertaking a comprehensive assessment, formulating individual management and treatment plans, and, using a wide range of treatment skills and options to provide a specialised programme of care.
- To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, in order to promote understanding of the aims of lymphoedema management, and to ensure a consistent approach to the care of the patient.
- To provide choice for patients as to how they receive their care, e.g. use of digital technology/on line platforms.

at Homes core service is our skilled team of Registered Nurses and Healthcare
Assistants who provide exceptional care and support in peoples last year of
life, at end of life, also providing additional services such as Lymphoedema,
Bereavement and Family Support, Befriending, Occupational Therapy and
Complementary Therapy.
We
provide individualised care and support to improve quality of life and our end
of life care helps facilitate a peaceful and dignified death in the home and
support to those who are bereaved.
Our
clinical service area covers a mixture of urban and rural communities, a region
of approximately 1,500 square miles throughout North and East Cumbria.

by cancer or cancer treatments but there are also other causes such as trauma,
infection, obesity or congenital abnormalities.
The
post-holder will support the Lymphoedema Nurse Specialist in planning,
delivering and evaluating the service provided to Lymphoedema patients, on a
day to day basis. The post-holder will deputise for the Lymphoedema Nurse
Specialist in his/her absence.
The post holder
will take a major role in the advanced assessment and treatment of patients
within the speciality who may have complex and/or chronic presentation. The
post holder will be expected to determine clinical diagnosis and implement a
management plan as an autonomous practitioner.

will be able to withstand the sustained physical effort that the assessment and
treatment of Lymphoedema demand.

will be expected to deal sensitively with patients/carers who have high levels
of anxiety and who may have difficulty communicating effectively.
