Job description

Job responsibilities

  • To deliver emotional support, advice and guidance via face to face, telephone, and video.
  • To assess a situation to determine risk and the needs of the person accessing support.
  • To make decisions under pressure whilst communicating with the person accessing support.
  • To accurately record new contacts and details of visits and calls on relevant electronic systems.
  • To work in line with performance targets and key contractual requirements, providing data and case studies as requested.
  • To advise and be actively involved in individual risk assessment and review.
  • Where identified, an appropriate action plan/safety plan will be put in place to meet individual's needs To encourage those accessing support to recognise, understand and manage factors that affect their mental wellbeing and physical health To work with a range of partners and external agencies to meet individual's needs and support with individual safety and delivery of the service
  • To work as a team member, sharing information and supporting colleagues, ensuring they have the knowledge, information, and resources to be successful in their roles and make a positive and valued contribution to the service.
  • To communicate with external stakeholders as required. To actively obtain relevant literature / resource materials from appropriate internal teams and external agencies to assist with support conversations and signposting. To partake in training and mentoring for all new staff.
  • To attend all relevant meetings and supervision sessions.
  • To be able to work as part of a team and on own initiative where required.
  • To work with the team to continually improve own and team performance.
  • To ensure your personal training and development needs are identified.
  • To actively involve people and their informal support networks in service planning, delivery and review. To carry out duties on a rota system to meet the service requirements of being open 365 days a year, or as stipulated in area specific contract. To agree to be contacted whilst not on shift to alleviate any rota issues / difficulties that will assist in ensuring continuous service delivery (last option). To actively promote a positive image of Mental Health Matters and mental health issues. To engage with community events, meetings and forums and local organisation and networks To promote the service and MHM To proactively contribute to continuously improving the service by making positive suggestions, providing constructive feedback and assisting in the implementation of agreed new ways of working. Any other duties relevant to the post.

    • Level 2 qualification in Health and Social Care, or equivalent subject, and be willing to complete an appropriate Level 3 qualification within 12 months of employment

    Experience

    • Have awareness of/lived experience and
    • experience of working with people who have experienced mental health issues
    • Experience of record keeping
