About Us
IMP Primary Care Network is a collaborative group of 9 GP practices serving approximately 74,000 patients across Lincoln and surrounding villages. We are committed to delivering high-quality, person-centred care, supporting our population to live healthier lives and ensuring our care home residents receive proactive, coordinated and compassionate healthcare.
This new and exciting role will suit an experienced GP with an interest in the Enhanced Health in Care Homes service.
The role will involve 1-3 sessions per week.
Our EHCH team is well established with excellent relationships with our Care Homes. The team consists of an Clinical Pharmacist, Nurse Practitioner, Pharmacy Technician and Care Coordinator. Our current GP, who set up the service, is looking to take full retirement.
Main duties of the job
The Role
We are looking for an enthusiastic and motivated GP to join our Enhanced Health in Care Homes (EHCH) team. You will play a key role in improving outcomes and quality of life for residents in local care homes by providing proactive, holistic care as part of a multidisciplinary team.
The role includes:

Providing regular clinical sessions for care home residents under the EHCH framework.

Undertaking proactive ward rounds and clinical reviews.

Supporting personalised care planning and anticipatory care.

Working closely with care home staff, community nursing, pharmacy, and mental health colleagues.

Contributing to the reduction of avoidable hospital admissions and improving continuity of care.

Supporting the delivery of high-quality end-of-life and palliative care.

Job Purpose
To provide high-quality, proactive and person-centred medical care to residents of care homes as part of the Enhanced Health in Care Homes (EHCH) service within IMP PCN.
The post holder will work as part of a multidisciplinary team to improve outcomes and quality of life for care home residents, reduce avoidable hospital admissions, and support continuity of care.
Key Responsibilities
Clinical Duties

Deliver proactive and reactive medical input to designated care homes as part of the PCN EHCH model.

Conduct regular structured ward rounds and review new residents.
-
Participate in comprehensive geriatric assessments and personalised care planning.
-
Support anticipatory care and end-of-life care planning.
-
Provide clinical leadership in medication reviews and optimisation (in collaboration with PCN pharmacists).
-
Contribute to the early identification and management of frailty and long-term conditions.
-
Provide urgent clinical input when required, in partnership with the wider MDT and the residents usual GP practice.
Teamwork and Collaboration
-
Work collaboratively with PCN colleagues, care home staff, community nursing teams, pharmacy, mental health, and social care professionals.

Attend multidisciplinary team (MDT) meetings and case conferences.
-
Develop and maintain strong relationships with care home managers and staff.
-
Support effective communication between the PCN, practices, and other healthcare providers.
Quality and Governance

Contribute to the continuous improvement of the EHCH model within the PCN.

Support the collection and reporting of relevant data and outcomes.
-
Participate in clinical audit, significant event reviews, and quality improvement initiatives.
-
Ensure all clinical activity is recorded accurately and in accordance with PCN and practice policies.
Education and Development

Keep up to date with relevant clinical developments, NICE guidance, and best practice in care of older people.

Participate in clinical supervision, peer learning, and appraisal processes.

Support the development and training of colleagues and other team members where appropriate.
