Job summary

To support the Ageing Well Team, working as part of the PCN multi-disciplinary team, delivering care within their scope of practice to the House bound population of the PCN.

The Healthcare Assistant will deliver and assist the Ageing well service, by carrying home visits to assess patients needs, complete PCSPs provide treatments such as phlebotomy, observations, diabetic foot checks. They will also look at preventative care, health promotion and patient education.

Main duties of the job

a. Undertake new patient assessments for the Ageing well service. (PCSPs)

b. Carry out baseline observations such as pulse oximitery, blood pressure, temperature and pulse rate, recording findings accurately

a. Optimising the home visit by checking for and completing QOF work within scope of practice

b. Liaise with the PCN team about patient needs / share at MDT

a. Carry out ECGs as requested, and action results as per protocol

Deliver opportunistic health promotion where appropriate

Job description

Job responsibilities

The following are the core responsibilities of the Healthcare Assistant. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Undertake new patient assessments for the Ageing well service. (PCSPs)

b. Carry out baseline observations such as pulse oximitery, blood pressure, temperature and pulse rate, recording findings accurately

c. Facilitate routine and 24-hour BP monitoring, advising patients accordingly

d. Carry out BMI checks as directed

e. Act as a chaperone as required

f. When trained, undertake venepuncture

g. Check Abtrace for blood requests

h. Optimising the home visit by checking for and completing QOF work within scope of practice

i. Liaise with the PCN team about patient needs / share at MDT

j. When trained, administer flu / covid vaccinations

k. Carry out ECGs as requested, and action results as per protocol

l. Deliver opportunistic health promotion where appropriate

m. When trained, carry out diabetic foot checks

n. Participate in PCN audit as directed by the audit lead

o. Ensure fridges are cleaned routinely in accordance with extant guidance

p. Ensure fridge temperatures are checked and recorded daily

q. Carry out regular stock takes, check dates and order stock as needed

r. Participate in local initiatives to enhance service delivery and patient care

s. Support and participate in shared learning within the organisation
