Job summary

The Remote Monitoring Care Navigator role is a position based within EBPCs Headquarters in Bracknell. The role works within the Remote Monitoring shift pattern covering a 9.5-hour period with 30-minute unpaid lunch break.

By using digital healthcare technology, EBPC can better support individuals within their own communities. There is wide recognition of the need to embrace new ways of working and remote monitoring can ensure patients receive the most appropriate care within their own communities.

Information provided by patients is used to assist in decisions around care and viewable to those who need to see it. Patients will answer a set of questions specific to their need or condition using an app on a smart device. The answers to those questions will be sent to the EBPC Remote Management team who monitor the readings and symptoms and will contact the patient if they need support. We are using Remote Monitoring technology to help alleviate the current pressure on Primary Care.

Main duties of the job

As part of the Remote Monitoring Team, the Care Navigators role requires you to maintain service continuation ensuring The Remote Monitoring Team at EBPC continues to provide a successful and high-quality service to the NHS.

The role entails introducing and communicating with patients who are enrolling onto the Remote Monitoring Service. As well as explaining the service to the patient and explaining how the app (Docobo) and equipment works. The Remote Monitoring Care Navigator plays a vital role to EBPCs Remote Monitoring Service success; answering phone calls from patients and assisting with any queries or concerns the patient may have. The Remote Monitoring Care Navigators will work alongside our Clinical Support Team Members and our Nurse/AHPs.

Job responsibilities

To ensure the continuation of effective and caring service provided by EBPC with the following:

- Receive and process incoming telephone calls from patients and other members of the Remote Monitoring team

- Contribute to the onboarding of patients with short- and long-term conditions to the Docobo patient management platform

- Contribute to the discharging of patients with short- and long-term conditions from the Docobo patient management platform

- Assisting and liaising with clinicians on shift to ensure all patients within the service are being dealt with in a timely manner

- Liaising with the Team Leader where appropriate with any issues and escalating service critical issues

- Logging all issues and queries on shift in an accurate and prompt manner, and reporting to the appropriate personnel or department with high communication skills

- To always follow protocol and procedure within shift

- To ensure all statutory and mandatory training modules are kept up to date

OTHER REQUIREMENTS

EBPC Remote Monitoring Care Navigator role requires use of multiple systems for data entry, monitoring and other means required for service continuation. Therefore, this post requires a strong IT user background or experience with using software packages in the past, or someone who is comfortable with computers and software.

Despite previous telephone experience not being essential for the role we do require someone with a friendly and calm manner, and someone who can provide empathy to incoming callers.

As a provider to the NHS, it is company policy that all mandatory online NHS training modules are completed prior to the successful applicants first rota shift and for the applicant to maintain compliance levels on an annual or biannual basis depending on the modules.

The Remote Monitoring Care Navigator post is a full-time, where you will be given a set shift pattern. Additional ad-hoc shifts are available to all contracted or non-contracted staff and changes to your shift pattern can be made whether its to increase, decrease or change your shift pattern. However, changes will only come into effect should it be authorised by management and the Resources Department.
