Job summary
Working in close liaison with the multi-disciplinary team, to provide individualised physiotherapy interventions and advice for inpatients and wellbeing service users, who fall into those services criteria. To focus on specialist palliative care symptom management, enablement or end of life care. To also provide advice on issues relating to moving and handling.
Main duties of the job
Organise and administer the day-to-day physiotherapy needs of patients within the hospice in line with prescribed physiotherapy standards.
Assess and plan treatment programmes with patients and other disciplines, to attempt to maximise patients potential, taking into account any predictable deterioration.
Set realistic person-centred goals around wishes and aspirations of the patient, considering physical, emotional, social and spiritual aspects of their life.
Respond quickly to assess and review patients needs, constantly adapting to manage changing and evolving circumstances.
Support the patient and family /carers both emotionally and practically.
To adjust to changing physical abilities.
Assess for appropriate equipment required by patients at their home, in liaison with other members of the Family Care team.
Liaise with outside agencies to ensure optimum and timely referral and assessment of patients by those agencies.
Manage the assessment of wheelchair referrals and timely provision.
Record interventions with patients/service users in a timely and accurate format, whilst meeting all legislative requirements for record keeping.
To loan equipment for comfortable discharge planning whilst optimising quality of life and independence, according to individual needs.
To provide advice and signposting to community services for day service attendees.

Over the years the day care unit developed initially into a day therapy unit and ultimately into a drop-In centre providing therapies, information and care for patients, carers and bereaved families.
