Job summary

Working across West Northumberland you will be responsible for working collaboratively with community services, NHS colleagues and other providers, to ensure that palliative and end of life patients and their carers are able to access appropriate services.

We offer a generous salary and your expenses for travel will be reimbursed at £0.45 per mile.

You will be independently mobile and will be travelling within West Northumberland, working in GP practices, patients own homes and other community facilities.

Main duties of the job

You will help identify patients with palliative and end of life care needs who would benefit from care services, improve timely support for patients and families, work collaboratively with other health care professionals and improve communication within the community teams.You will act as an advocate for patients and carers, signposting to appropriate services as well as providing direct care

You will be working alongside our Hospice Registered Nurses, Hospice Support Workers, Pre and Post Bereavement Family Support Practitioners and Complementary Therapist to offer wraparound care to patients with palliative and end of life care needs and their carers.

Job responsibilities

Job Title: Link Nurse (Registered Nurse)

Hours: 24 hours per week. Flexibility offered in start and finishtimes and days of week

Salary: £23,899 plus travel costs of 45p per mile

Job Purpose:To enable palliative and end of life patients to access services and support in a timely manner, including practical, emotional and clinical care

Responsible to:Head of Care

Accountable to: Chief Executive Officer

Overall Aim:

The role is to inform and enable patients who have palliative and end of life needs and are currently on the GP palliative care register and their carers to access appropriate services. The post holder will work in collaboration with a defined group of GP practices within West Northumberland to develop and integrate services to provide timely access for patients with palliative and end of life needs. This will promote earlier referral to community and hospice services, which will reduce inappropriate admissions and lengthy stays in acute settings.

Main Duties and Responsibilities

  • Manage own caseload of patients working closely with community teams and Tynedale Hospice at Home Care co-ordinators to support consistency of care given.
  • Attend and contribute to regular MDT meetings within GP practices and wider based meetings when appropriate to end of life.
  • Identify and communicate with patients on GP palliative care register who are not accessing care services who would benefit from input.
  • Deliver education and training for staff in delivery of evidence-based end of life care.
  • Assess, plan, implement and evaluate nursing care provision to meet individual patient needs.
  • Provide end of life and palliative care training and expertise within the defined group of GP practices in West Northumberland.
  • Undertake nurse prescribing functions within the defined scope of practice.
  • Audit effectiveness of end of life and palliative care nursing interventions.
  • Demonstrate continuous service improvement, through learning from patient feedback and incidents.
  • Work collaboratively with the wider team to promote the earlier introduction of wraparound Hospice services.
  • Promote and develop the use of wraparound services within current wellbeing hubs.
  • Undertake the role of Practice Supervisor/ Assessor to nursing and medical students.
  • Contribute to THH strategy, developing services to patients with palliative and end of life needs and their families, auditing hospice services to identify areas of development and improvement.
  • Contribute to collection and reporting of appropriate Key Performance Indicators for palliative and end of life care.
