This is a very exciting time to be involved with Social Prescribing. It has been identified as a key component of NHS Englands Universal Personalised Care objective contained within their Long Term Plan, as well as Londons Health Inequalities Strategy for healthy communities published by The Mayors Office in 2018. Both strategies recognise the influence of social, emotional, economic and cultural factors on health and well-being and the importance of community connections to improving health and wellbeing, self-care and self-management of long term conditions.

Family Action is proud to be the provider of the existing local Social Prescribing Service commissioned by City & Hackney CCG delivering since 2014, which recently won Best Larger Project at the inaugural Social Prescribing Awards 2019. We are now delighted to be expanding the provision through additional funding provided by City & Hackney Primary Care Networks/Neighbourhoods.

Main duties of the job

You will work as part of a Primary Care Network Multi-Disciplinary Healthcare Team, providing 1:1 personalised support to Children and young people, individuals, and families utilising a holistic and strength-based approach, to help identify the wider issues that impact on their health and wellbeing and work together to co-produce a wellbeing plan to address these needs. You will have excellent holistic assessment skills, as well as interpersonal and communication skills that enable you to build trusting relationships which support individuals feel empowered with their wellbeing.

You will forge strong links with local VCSE organisations, community and neighbourhood networks, building on what is already available, to create a menu of community groups and assets to connect individuals to as part of their wellbeing plan.

You must be a good listener and have experience of working positively with individuals facing complex, social and emotional challenges that impacts on their wellbeing. Being creative as well as having a solution focussed attitude to removing barriers to health and wellbeing is essential.

You will also have strong leadership qualities , experience of supervising staff and report writing to support strategic and organisational requirements

Job responsibilities

To undertake whole family/children / young people assessments / holistic assessments and co-design Health and Well-being Plans with service users, identifying support needs to ensure maximum engagement in improving health and well-being.

To provide service users with continuity and a co-ordinated experience of care, remaining point of contact throughout the individuals social prescription.

Work with CYP and families to co-produce a simple personalised support plan to address their health and wellbeing needs based on their priorities, interests, values and motivations including what they can expect from the groups, activities and services they are being connected to and what they can do for themselves to improve their health and wellbeing

Be proactive in encouraging self-referrals and connecting with all local communities, removing barriers particularly for those communities who may be under-represented in this service. Be a friendly source of information about health, wellbeing and prevention approaches.

Where appropriate, carry out home visits and introduce CYP/families to community groups, activities and statutory services, following up when necessary to provide additional support to assist them being able to engage.

Forge strong links and collaborative relationships with local VCSE organisations, community and neighbourhood level groups, utilising their networks and building on what is already available to create a menu of community groups and assets. To ensure information on sources of voluntary and community support are up to date at all times to enable effective and accurate signposting and linking of service users with services.

As part of the PCN multi-disciplinary team, build relationships with staff in GP practices within the local PCN, attending relevant MDT meetings, giving information and feedback on CYP and Families Social Prescribing. Train and develop GPs and health teams knowledge on how to identify children, young people and families suitable for the service.

To work in partnership with all voluntary and community organisations to build a comprehensive database of local resources to design and support the Social Prescription Menu. To ensure information on sources of voluntary and community support is up to date at all times to enable effective and accurate signposting and linking of individuals with services.

Provide high quality supervision to staff, volunteers, and students, and monitor the safeguarding responsibilities and training of staff.

Act as safeguarding lead for the CYP and Families Social Prescribing service and work according to Family Actions and City and Hackneys safeguarding policies and procedures, safeguarding Vulnerable adults and children in collaboration with statutory and voluntary agencies escalating cases when required.

To carry out case audits on a regular basis, complete audit reports and share information with the SP worker/s and the Hackney services manager.

Set up and maintain comprehensive data and evaluation systems, including individual health outcome tool (e.g. Wellbeing star and EQ5D).

Attend the steering group including representation from all stakeholder groups to support the on-going development, monitoring, and evaluation of the programme.

Provide quarterly comprehensive outcome focussed reports detailing the progress of the service, including budgets. Sharing report findings and promoting the service at key strategic meetings and other stakeholder forums, scoping developmental opportunities for the CYPSP service with other PCNs and wider statutory and VSC partners.

To recruit, train and supervise volunteers, matching them to individual service users for specific time-limited support in achieving positive engagement in activities promoting health and well-being.

To keep records of your work and adhere to confidentiality, information sharing protocols and provide monitoring information as required.

To implement the principles of Family Action and Hackneys Equal Opportunities Policy in every aspect of your work and positively promote the principles of the policy amongst colleagues, service users and other members of the community.
