Job summary

The Integrated Family Healthcare GP Federation is looking for a compassionate and dedicated ADHD Service Navigator to join our team working on our new ADHD pilot project in Hastings. This new service will offer an alternative for patients age 16+ awaiting a diagnosis of ADHD, under the right to choose pathway.

Main duties of the job

The Service Navigator will have a key role in supporting our patients through the patient pathway. We are looking a highly organised individual who is confident in managing patient workflow and has strong interpersonal skills to engage effectively with patients. The care coordinator will need to ensure smooth booking of appointments, completion of questionnaires and assessment forms, and communication between the patient and the service.

The Integrated Family Healthcare GP Federation's primary mission is to support the delivery of the Directed Enhanced Service (DES) through the Hastings & St Leonards Primary Care Network (PCN).

We comprise of 8 general practice surgeries and medical centres across Hastings and St Leonards, serving a population of 100,000 patients.

Job responsibilities

Patient Support

  • Act as the named coordinator and first point of contact for patients progressing through the ADHD pathway via telephone or in person.
  • Book appointments into the clinical system.
  • Assist patients in completing assessment forms and patient questionnaires in a clear manner.
  • Monitor attendance and proactively arrange follow up appointments with patients who may have missed or cancelled appointments.
  • Provide professional and empathetic explanations to patients about the process.
  • Signpost patients to wraparound support following a diagnosis (eg webinars) and assist with any barriers to access with the patient ensuring they receive the support they need.
  • Communicate effectively between clinicians, GP Practices and wrap around care providers.
  • Track patient progress and escalate patients who are not progressing through the service.

Administrative Support

  • Accurately enter patient information and input data into the clinical system.
  • Provide administrative support to clinicians and service delivery manager, including diary management and rota coordination.
  • Assist in the coordination and scheduling of multidisciplinary team (MDT) meetings.

Information Governance and Systems Management

  • Accurately enter and update patient data in clinical systems
  • Handle confidential information with discretion and professionalism in line with confidentiality and information governance policies
  • Support service audits, performance reporting, and data collection

Service Development

  • Contribute to processes and systems that improve patient experience and flow
  • Identify gaps or inefficiencies and work with the team to resolve them
  • To proactively identify and address day to day operational issues as they arise.

  • Excellent organisational and time management skills with the ability to prioritise workflow effectively.
  • Empathetic and compassionate approach towards patients and their families
  • Ability to work independently and as part of a collaborative team.
  • Proactive work style with the ability to solve problems autonomously.
  • Previous experience of working with neurodivergent individuals, including people with ADHD, or in a role in supporting patients with mental health needs.
  • Ability to recognise and respond to different communication needs, adapting where necessary to support effective engagement with patients.

Desirable

  • Understanding of ADHD and its impact on executive functioning
  • Knowledge of EMIS or other NHS systems (training will be provided)
  • Experience in supporting neurodivergent patients or with mental health needs.
