Do
you have a passion for digital? Have you ever thought about taking that passion
and combining it with your skills and knowledge to support the health and
wellbeing of your community?
To address the ever-increasing demand for healthcare
services and to better support individuals within their own communities, there
is wide recognition of the need to embrace new ways of working and digital
healthcare technologies to ensure all patients receive the right care, in the
right place, at the right time.
We are looking for someone to join our existing team working 22.5 hours per week at our PCN base within Ludlow Youth Centre. Driving licence and access to a car is essential for the role.
Main duties of the job
The role of the Digital Care Coordinator will be to

to NHS organisations, and involves using available data sources to understand
the health and well-being profiles of our residents. It means working within a
team of PCN and practice staff to support their activities with the relevant
data and data analysis, and it means helping to develop our digital maturity.
But above all it means making a positive difference to the lives of our
residents, by highlighting their needs and shining a light on hidden health
inequalities.
