Job summary
We are seeking Clinical Pharmacists (1.5 WTE) to work across practices within the South Birmingham Alliance Primary Care Network (SBA PCN), the practices are based within B30, B31 & B38.
We are looking for Clinical
Pharmacists who are already qualified independent prescribers, and who have ideally completed the CPPE Pathway.
This is an exciting opportunity to join our existing team of Clinical Pharmacists and other Allied Healthcare Professionals.
Professional Leadership will be provided by the Consultant Pharmacist at OHP and the PCN Clinical Director.
Main duties of the job
The position of Primary Care
Network Clinical Pharmacist has an essential role in improving the healthcare
we provide to our patients. We are recruiting for a clinical pharmacist to work
with existing practice and PCN teams to deliver consistency and excellence in
medicines optimisation across the member practices.
We are looking for pharmacists who will work as part of a multi-disciplinary team, developing and managing
medicines optimisation services within our practices. Demonstrating
improvements in value and patient outcomes and contributing to reducing health
inequalities through the safe and appropriate use of medicines. This includes
implementation of systematic processes for improving medicines optimisation
across our PCNs and the wider OHP network. Work programmes will be prioritised
according to the population needs of the PCN.

Health Partnership was set up by local GPs who are passionate about providing high
quality primary care and using their time and skills effectively to benefit
patients.
We are currently a GP
partnership of 30 practices with 39 sites, serving around 280,000 patients
in Birmingham, Wolverhampton, and Shropshire.

partnership offers a shared administrative and management structure, cutting
down the time doctors have to spend on admin. It opens up economies of scale to get best value from budgets and can access new funding streams that are only available to large GP organisations. It has the resources to develop innovative

The post holder is a clinical pharmacist, who acts within their
professional boundaries, supporting and working alongside a team of Health Care
Practitioners within general practice and a Primary Care Network (PCN).
The post holder will provide primary support to general practice staff
with regards to prescription and medication queries. They will perform medicines
reconciliation on transfer of care and implement systems for safer prescribing,
providing expertise in clinical medicines advice while addressing both public
and social care needs of patients in the PCN.
The post holder will work as part of a multi-disciplinary team in a
patient-facing role. They will take responsibility for areas of medicines optimisation
within the PCN and undertake structured clinical medication reviews to
proactively manage patients with complex polypharmacy.
The post holder will provide clinical leadership on medicines optimisation
and quality improvement (QI) and manage some aspects of the quality and
outcomes framework and enhanced services at practice and PCN level.
The post holder will work with all sectors of pharmacy across the health
system and the wider OHP organisation to improve population health.
Demonstrating improvement in patient outcomes and working to reduce health
inequalities across the PCN contributing to improving the efficiency of general
practice and delivering a best in class service.
The post holder will be supported to develop their role in line with the
OHP workforce strategy, this will include becoming and operating as a non-medical
prescriber. This role will be reviewed at least annually, where
the Job Description may be updated in line with the needs of the organisation.
