Our Health Partnership are looking to appoint a Clinical Pharmacist (0.6) for 3 days per week. This is an exciting opportunity to join our team at Moseley, Billesley & Yardley Wood Primary Care Network (MOBY PCN) - working at practice/s within B13, B14, B27 & B28.
The position of Primary Care Network clinical pharmacist has an essential role in improving the healthcare we provide to our patients. We are recruiting for a clinical pharmacist to work with existing practice and PCN teams to deliver consistency and excellence in medicines optimisation across the member practices.
The successful candidate will be part of a dynamic and forward-thinking team of Clinical Pharmacists and other Allied Healthcare Professionals employed by Our Health Partnership, working across the MOBY PCN. Professional Leadership will be provided by the Consultant Pharmacist at OHP and the Lead Clinical Pharmacists in the PCN.

Main duties of the job
We are looking for a pharmacist who will work as part of a
multi-disciplinary team, developing and managing medicines optimisation
services within our practices. Demonstrating improvements in value and patient
outcomes and contributing to reducing health inequalities through the safe and
appropriate use of medicines. This includes implementation of systematic processes
for improving medicines optimisation across our PCNs and the wider OHP network.
Work programmes will be prioritised according to the population needs of the
PCN.

Health Partnership was set up by local GPs who are passionate about providing high
quality primary care and using their time and skills effectively to benefit
patients.
We
are currently a GP partnership of 33 practices with 42 surgeries. 176 GP
partners and circa 80 salaried GPs in Our Health Partnership serving around
300,000 patients in Birmingham, Sutton Coldfield, Wolverhampton, Solihull and
Shropshire.
The
partnership offers a shared administrative and management structure, cutting
down the time doctors have to spend on admin. It opens up economies of scale to
get best value from budgets. It has the resources to develop innovative
services and effective partnerships with local hospitals and care services. And
it can access new funding streams that are only available to large GP
organisations.

The post holder is a clinical pharmacist, who acts within their professional boundaries, supporting and working alongside a team of Health Care Practitioners within general practice and a Primary Care Network (PCN).
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will perform medicines reconciliation on transfer of care and implement systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the PCN.
