Job summary
An exciting new post has been created to support the well-established
SPACE (Specialist Paediatric Palliative care) team based at Shooting Star
Children Hospice and develop a local specialist paediatric palliative care
presence in Sussex. This combined post supports the complexity of referral
pathways across large geographical footprints from specialist palliative care
and from large paediatric interdependent services. These interdependent feeder
services include the regional foetal medicine service and tertiary neonatal,
high dependency and intensive care units The post seeks to strengthen local
specialist palliative care provision and aligning the perinatal palliative care
program and paediatric tertiary clinical pathways across Surrey, Southwest
London and Sussex.
Main duties of the job
This is a new,
substantive full-time post in paediatric palliative medicine to manage the
increasing clinical activity for the SPACE service and access to specialist
paediatric palliative care consultant for Sussex. The post is suitable for a
consultant with CCST training in paediatric palliative medicine or equivalent,
or within 6 months of completing training.
The post will be based at the Shooting Star Childrens Hospice (3 days)
and Chestnut Tree Hospice (2 days). Due to the nature of the work, supporting
preferred place of care and end of life, the post holder will provide frequent
outreach work into other hospitals and the community including home, hospice
and educational settings.
The successful
applicant will be part of the specialist consultant and multi-disciplinary team
providing care for children and young people requiring specialised symptom and
palliative care support.

childrens hospice is a charity providing care to babies, children and young
people with life limited and life threatening conditions and their families
living in West Sussex, East Sussex and South East Hampshire. The hospice
supports children and families through the trajectory of their condition by
providing paediatric palliative care including respite and end of life
provision. This post will be the first locally based specialist consultant post
in the Sussex region.

The post holder will be an expert clinician in pain and symptom management, end of life care, advance communication and advance care discussion and planning. They should be able to deliver paediatric palliative care across all clinical settings.
The post will support:
- Specialist non-oncology paediatric palliative care covering the geographical catchment area of the Shooting star childrens hospices SPACE team- Surrey and Southwest London.
- Provision of a specialist consultant consultancy and outpatient clinic service for CYP with specialist paediatric palliative care needs in Sussex geographical region (Sussex service).
- Out of hours telephone specialist advise service through participation on the PATCH oncall rota (separate contract).
The post holder will for SPACE and COAST teams:
- Lead on perinatal palliative care regional program for the SPACE team and Sussex
- Attend the regional foetal medicine meeting (virtual or in-person)
- Engage in education and teaching of staff and local services
- Participate in Morbidity and Mortality meeting and the child death review process
- Participate in audit and service evaluation projects
- In-reach into paediatric tertiary services to support clinical service delivery and pathways
- Carry a clinical case load
- Review of patients in all care settings (hospital, home, hospice and education)
- Introduce, review and update of symptom and emergency care plans
- Introduce, review and write advance care plans
- Support complex decision making
- Provide specialist medical advice on symptom, palliation and end of life care
- Complete death summaries
- Should maintain Good clinical practice (GCP) training and participate in paediatric palliative care research
- Participate in ongoing paediatric palliative care research
- Maintain clinical expertise with engaging with local, national and international projects as appropriate.
The post holder will for SPACE team (SSCH):
- Attend the weekly paediatric PATCH meeting (if contributing to PATCH oncall rota)
- Attend the monthly SPACE MDT meeting
- Support the GRID/SPIN trainees in Paediatric Palliative Medicine
- Attend the medicine management meeting (quarterly)
- Provide a fortnightly hospice clinic
The post holder for the COAST team(CTH):
- As part of providing regional specialist service, engagement and clinical inward reaching support is required particularly to subspecialist paediatric services with identified potential high referral numbers including NICU and HDU.
- Run a fortnightly hospice clinic
- Support development of a specialist paediatric palliative care service including data collection, service design and delivery
- Support the development of specialist team especially the Clinical nurse specialist roles
The post holder may contribute to the PATCH oncall service which is a separate contract. This will either be a 1 in 4 second tier post or 1 in 6 first tier post depending upon vacancy availability.
The post holder is expected to develop excellent working relationships with the teams interdependent on paediatric palliative care services.
Line management, support and mentorship for the post holder will be provided by the Medical Director and Paediatric Palliative care lead Dr Anna-Karenia Anderson.
