Job summary
This role has been developed within Shooting Star Childrens Hospices to ensure proactive admissions and discharges across the Hospice working alongside the multidisciplinary team.

This role has been developed within Shooting Star Childrens Hospices to ensure proactive admissions and discharges across the Hospice working alongside the multidisciplinary team.

The individual in this role will work in partnership with internal and external professionals involved in the care of children, young adults and families.
c) Scope of job
The post holder will:
Act as key co-ordinator for admissions and discharges to the in-house service in line with local and national policies.
Co-ordinate all appropriate procedures working with the Multi-Disciplinary Team (MDT), outside agencies, young people and their families ensuring that all plans are in place for the time of admission and discharge.
Establish process and monitoring systems to improve admissions, cancellations, and discharges.
Active participation in development of standards, policies and guidelines, and their subsequent audit and on-going development.
a) Main duties and key responsibilities
Admissions and Discharges
To facilitate safe admission and discharge in partnership with the bookings team, statutory funding manager, wider MDT, outside agencies, child and young people and their families ensuring all plans are in place for the time of admission and discharge including transport arrangements.
Ensure effective communication with MDT by attending daily huddle, weekly MDT, bookings, and handover meetings.
Facilitate admission and discharge and early MDT assessment.
Establish collaborative working relationships with referrers to ensure effective and timely admission and discharge.
Ensure that carers/parents have information relating to admission and discharge.
Assist with complex admissions and discharge liaising with CHC/Social care to ensure timely provision of assessment and documentation.
Liaise with the practice education team regarding children with complex care needs to ensure that staff are appropriately trained prior to these admissions.
Act as a resource to staff for evidenced based admission and discharge practice.
Establish monitoring systems to highlight reasons for delays in admissions, cancellations, and discharges.
Contributes to Hospice audit process.
Ensure that all MDT members have admission and discharge procedures training.
Work with the specialist palliative care teams to ensure parallel planning is in place for all end-of-life care admissions.
There will be times when the job holder will be expected to support clinical practice In-House, so the post holder has a working knowledge of how admissions and discharges are managed on the floor.
Safeguarding
To work collaboratively with partner agencies across health, social care, education, and others to safeguard children and adults at risk and promote their welfare.
MCA Dols
To work with the Safeguarding Lead on statutory compliance in respect of DoLS/LPS for in-house admissions.
To ensure that standards are appropriately applied so that patients are cared for in the least restrictive manner within the options available to them.
To ensure that all DoLS/LPS applications are completed and processed.
Advisory
To identify new ways of working in response to national drivers and local need. To challenge decisions when appropriate and implement agreed actions.
To report to Heads of Care and Directors any potential risk of non-compliance.
To contribute to the development of policies and protocols that facilitate safe, effective, and timely information sharing processes, reflecting current legislation and national policy.
To participate in bookings meetings, MDT, journal club, study days and teaching sessions.
Education
To facilitate learning and increase self-awareness of resilience and burnout.
To work with the Practice Education and Quality Team to develop and deliver training.
To maintain personal and team knowledge of safeguarding legislation including the MCA, Gillick competency and DOLs national, regional, and local procedures and practices.
Governance
To work alongside the Head of Hospice Services, Lead Nurses and Statutory Partnership Team to ensure effective policies, procedures and systems are in place in accordance with legislation and statutory and regulatory requirements.
To actively undertake clinical audits and monitor and collects statistical data in relation to admissions, discharges, cancellations and MCA/DoLS/LPS
Ensure that the Director of Care, relevant committees, and groups are briefed on issues relating to admissions and discharge, particularly where a case may present serious concerns for SSCH.
To produce reports and presentations as required, indicating themes, key developments, and risks.
To investigate internal incidents and attend multi-agency reviews, ensuring effective systems are in place for learning and sharing lessons both internally and with statutory partners.
To contribute to service evaluation to ensure equity of service and maintain high quality service provision.
To consider ways in which technology can enhance the work of SSCH and to propose developments.
Leadership
To ensure effective and safe admission and discharge of babies, children and young people.
To establish and build professional relationships with professionals/networks that progress effective admission/discharge.

Utilise the Care database reporting system to provide data for quality audits and research.
