Job summary
The Integrated Neighbourhood Team MDT Co-ordinator Team Leader will facilitate the personalised delivery of care for complex and
long-term condition patients. They will focus on personalised care plans and demonstrate knowledge of local resources that can help the patient self-manage where
possible.
We recommend 1 WTE Band 6 MDT Coordinator Team
Leader to cover six Merton practices to meet the needs of the local population
and to ensure the safe and high-quality care of complex patients.
The post holder would be expected to have
responsibility for a specific Integrated Neighbourhood Team / cluster and for
specific MDT meetings, acknowledging that the size and needs of each cluster is
varied. They will be expected to cover other areas for periods of leave and
ensure that there is support during weekday core working hours, liaising across
practices to ensure MDT attendance from the co-ordinators and other key members
of the MDT.
Main duties of the job
Ensure successful multidisciplinary working by
planning, coordinating and facilitating MDT meetings, ensuring attendance and
outcomes within each practice within an INT, working with all partners relevant
to the MDT, including practice administrators to support this.
Provide administrative support before, during
and after the meetings; including supporting the onward referral and
communication across agencies, key to this is having the most up-to -date
position regarding the patient from RIO and GP practice system.
Support the development and sharing of care
plans for patients with complex needs, frailty or end of life care.
Support data collection and audit of services,
working closely with stakeholders across health, social care and the voluntary
sector as appropriate to support review and improvements in delivery and
outcomes.
Attendance to 100% MDT Coordinator meetings is
essential.
Ensure a fair split of workload tasks.
To link with other services inside and outside
of meetings (e.g. HARI, MERIT, CLCH etc).
To support adequate preparation by all relevant
members for meetings (as agreed by the groups standard operating procedures and
Terms of Reference).
Maintain a good relationship with the MDT and
other potential representatives and contributors (e.g. secondary care, mental
health services).
Appropriate access to key IT systems should be
in place.
